Introduction Demographic profile: Name: MR. X Address: Zone 1 Sua, Camaligan, Camarines Sur Age: 27 yrs. Old Sex: Male Civil status: Single Nationality: Filipino Educational Attainment: Elementary level Religion: Roman Catholic Occupation: None Chief Complaint: cataract Present Treatment: None Past Hospitalization: None Patients History of Present Illness: Mr. X complains about pain at his left eye during our home visit. For the past 3 years he was a construction worker in Sta. Mesa, Manila in 2007, while he is working in construction feild it suddenly enter a foreign object (baybay) on his left eye then he suddenly felt pain. That was the time that his cataract started. There is no cataract in there family but his brother blind on one eye, their beliefs its a superstition Patients History of Past Illnesses: Patient does not have any history of past illnesses.

V. Gordons Functional Health Pattern A. Health Perception and Health Management B. Nutritional-Metabolic Patterns He eats his meal three times a day. He usually takes 5 meals a day (breakfast, lunch, dinner and 2 snacks). Regularly he eats rice, vegetables sometimes fish and meats. There are no allergies in food. C. Elimination Pattern There is no problem in bowel and bladder elimination. D. Activity-Exercise Pattern He doesnt have a regular exercise pattern. He works in the farm and he considers it as his exercise. E. Sexually Reproductive Pattern F. Sleep-Rest Pattern He rests through watching TV. He usually sleeps at 10-11pm then he woke up at 8 am. Coping Stress Tolerance His mother told us that irritability may be considered as factor which causes him stress. Whenever he feels irritated, what they usually do is to give him his writing material for him to draw or let him sleep to divert his attention. K. Value-Belief Pattern His strong determination to learn how to write and his will for education makes his life worthwhile despite the differences he has from other normal children. He always tell her mother to send him to school, learning is what he aims for. VI. Insight
