OFFPRINT TERMS OF DELIVERY & PRICES

1. These terms apply only to the authors of the article; only the corresponding author receives an order
form. It is essential that the offprint requirements of all the authors of a paper are included on this order
form, as offprints ordered after publication are charged at 50% extra.

2. Offprints can be ordered by filling in the Offprint Order Form and returning it (by fax or mail) to
Elsevier at the address given on the form. Please note that the minimum order is 50. Ordered offprints
will only be supplied if this form is completed and returned. Receipt of offprint orders is not
acknowledged.

3. Any correspondence in connection with offprints must state the name of the journal, title and our
reference number of the article (see the order form), and name of the corresponding author.

4. If applicable, your order should be accompanied by an Institution/Company Purchase Order; the

reference number of which must be specified on the Offprint Order Form.

5. Note that payment will be processed and an invoice raised after publication of the article.

6. Prices are subject to change without prior notice.

7. Please provide a street address for your offprints; courier services cannot deliver to PO Boxes.

8. Cancellation or return of an order will not be accepted after printing of the journal has been finalized.

9. Offprints will be delivered 4–6 weeks after publication and are dispatched by standard air services at
no extra charge. Claims for missing or damaged offprints must be made within six months of the date
of publication of the issue in which your article appears.

10. Customers in countries where VAT or Sales Tax is applicable will be charged for any tax due, and
registered numbers must be given on the order form. US Sales Tax or exemption numbers are also
required where applicable.
