An Advanced Assessment for Seniors


Healthways Senior Well-Being Assessment (WBA) delivers an insightful view of member health history, status, social and emotional health, and health-related behavior. Healthways provided input to CMS guidelines for a senior wellness assessment. Our CMS-approved, Senior WBA fully addresses star rating criteria. It combines our traditional, NCQA-accredited, health risk assessment with key well-being metrics leveraged from the Gallup-Healthways Well-Being Index. Questions cover six, scientifically defined areas of well-being and explore challenges specific to seniors: risk of falls, hearing and vision issues, the strength of social ties, and the ability to perform the instrumental activities of daily living. Members can complete the annual assessment online or in print with phone support. Colorful, easy-to-read reports show each person the risk factors and behaviors that are helping or hurting well-being. Aggregate reports for your total population compare metrics against national, regional, state, or local benchmarks from the Well-Being Index.

Reduce Big Risks with Proven Solutions


Obesity and tobacco use are two of the biggest risks for health care and cost. Our well-being improvement strategy includes proven solutions for both: QuitNet for tobacco cessation, and Innergy for healthier weight. Participants are identified for both programs using Senior WBA data. Developed in collaboration with major research university Johns Hopkins Medicine, Innergy brings a model for sustained weight loss, successfully tested in the Johns Hopkins POWER trial, to your population. (Nearly 40% of participants receiving intervention lost and maintained a loss of 5% or more of their initial body weight.4) The program includes frequent contact with a weight management coach and online social support, accessible via Well-Being Connect.

Personalized Well-Being Plans


Each person in your population receives a Well-Being Plan building on data captured in the Senior WBA. Both resources are integrated through Well-Being Connect, a guided online experience with an engaging, easy-to-use interface and extensive behavior change science behind the scenes.

Personal Well-Being Plans: Recommend behaviors for improvement and relevant steps to action, directly related to individual health risks. Allow members to focus on behaviors they are most motivated to change, to work on multiple behaviors at once, and to set and track personal goals. Integrate the full Transtheoretical Model (TTM) of behavior change through embedded technology and strategies, developed in partnership with Dr. James O. Prochaska and Pro-Change Behavior Systems, Inc. Beyond just the stages of change, action steps reflect individual self-confidence and decisional balance (views on the pros and cons of change). Link members to purposeful, relevant tools, articles and video segments, and health professionals.

Guided Online Improvement


Nearly 50% of adults age 65 and over use the Internet.3 Seniors are increasingly comfortable with online tools. Well-Being Connect is simple to access and update. It engages your senior population at every level of health. Guided improvement in behaviors like selfmanagement, appointment adherence, healthy eating, and weight management directly influence star rating measures. Well-Being Connect also offers: Highly configurable messaging to support member communication A flexible incentive platform, easily modified as CMS guidelines evolve, to accommodate effective use of member rewards Dynamic health trackers that encourage frequent interaction (tracking exercise, healthy eating, medication adherence, weight, and more) Access to social support features for tobacco cessation (QuitNet.com), weight management (Innergy), and more

Senior-Focused Telephonic Health Coaching


To further drive risk reduction, behavior change, long-term health care savings, and star rating gains, our well-being improvement strategy includes direct health coaching. We focus coaching support on members with high health risks and on improving specific star rating metrics. Our sophisticated identification process draws data from the Senior WBA and available claims and reflects the unique dynamics of senior populations. Healthways coaches for seniors are highly experienced health professionals who understand the aging process and related social and emotional needs. Coaches apply evidence-based knowledge and bring compassion to each call. They have access to all aspects of member health and program involvement: well-being risks, medications, conditions, needed tests or exams, and personal health goals. Each interaction builds on previous contact to support specific action plans. Call frequency and intensity reflect individual health status and change.

High-Risk Cost Avoidance


Add short-term impact to your well-being improvement strategy with Healthways High Risk Cost Avoidance. Through customized predictive models, we anticipate and prevent high-cost events by targeting highly personalized outreach to identified seniors. For those entering the end of life process, specially trained nurses can provide telephonic compassionate care counseling and education that improves quality of life and reduces costs.
