Original and Xerox copy of Form 138 (Report Card) currently enrolled as Fourth Year HS student - True Xerox copy of grades from 1st For 3rd grading period certified by the HS principal. 3. Original and Xerox copy of Certificate of Good Moral Character 4. Two (2) recent copies 2 x 2 ID picture. 5. Non-refundable testing fee of P300.00 Transferees 1. Filled up application form for Admission Test. 2. Original and Xerox copy of Transcript of Records/ Certification of Grades signed by the University or College Registrar. 3. Original and Xerox copy of Honorable Dismissal. 4. Two (2) recent copies 2 x 2 ID picture. 5. Original and Xerox copy of Certificate of Good Moral Character. 6.
