What this form is for


To help the Australian Government improve its services for migrants in Australia, we ask you to complete this form. The answers you provide are important and will help government agencies plan and co-ordinate services, such as English tuition, health and education, more effectively. In most cases the answers you provide will be converted into statistics. The information you provide may also be disclosed to agencies authorised by the Department of Immigration and Citizenship (the department) to receive information to assist in the provision of settlement services. Your personal information, including details of your latest known address, may be provided to the Australian Bureau of Statistics to facilitate demographic research on new arrivals and analysis of various migration programs and resultant settlement outcomes. Settlement location details you provide on this form will be used by the department (and other government agencies) only for the purpose of facilitating settlement services and cannot be accepted as notification of change of address under section 494B of the Migration Act 1958. The storage, use and disclosure by the department of the information you provide in this form is governed by the Migration Act 1958, the Privacy Act 1988 and the Commonwealth Crimes Act 1914.
