Patient Teaching Plan Anticoagulant Therapy - Coumadin PURPOSE: To provide patient diagnosed with DVT the necessary information to manage their anticoagulant therapy, specifically Coumadin GOAL: The patient will be able to verbalize their understanding of their medical condition and why anticoagulant therapy is required to manage their condition Objective Content Outline Method of Presentation Time Allotted ( in minutes) Resources Method of Evaluation

Following a 45 minute teaching session the patient will be able to: Client will be able to verbalize his understanding of his medical condition (DVT) and need for an anti-coagulant (Coumadin) to manage his condition (cognitive) Understanding of importance of INR testing and keeping appointments for INR blood tests

Explanation of DVT and how an anticoagulant works

1:1 instruction

Pamphlet on DVT Pamphlet describing how anti-coagulants work

Client verbalizes his understanding of DVT and need for anticoagulant therapy

Explanation of INR blood tests and the range that indicates blooding clotting capability is balanced

Instruction and Demonstration

Sample of INR test results; Calendar for client to track INR testing dates

Recognize Coumadin tablets

Sample of pills including Coumadin

Client will verbalize understanding of INR test and importance of testing on date and time ordered by physician; Client will fill out calendar for 1 month lab appts. Client will pick out Coumadin from within a sample of different pills

Client will verbalize his understanding of initial dosage regiment; Client will demonstrate by placing pills in correct slots in pill box Client will verbalize understanding of side effects and what to do if he experiences any of them

Client understands his initial dosage, days and time to take Coumadin

Document stating dosage, days, and time of day to take Coumadin for first week Weekly pill box

(cognitive) Client understands possible side effects and what to do if he experiences any of them (cognitive) Client understands risks of bleeding from falls or injuries (cognitive) Client understands importance of avoiding intake of excessive amounts of foods high in vitamin K (cognitive and psychomotor) Recognize what medicines and herbs can interact with Coumadin Review of side effects and what to do if client experiences any of these 1:1 instruction 7 List of side effects and procedures for follow up if any side effects are experienced

Explanation of both external and internal bleeding

Client verbalizes understanding of risks of excessive bleeding from falls or injuries Client will select those foods high in Vitamin K from a random list of foods

Explanation of how Vitamin K affects Coumadin

Explain interactions of medications, both prescription and OTC as well as herbs and supplements with Coumadin
