Tiza, Roxas City Dear Sir, We the graduating students of College of St. John Roxas De La Salle Supervised taking up Bachelor of Science in Nursing, as part of our requirements; we are conducting a research study entitled Socio-Demographic Profile and Family Planning Practices of Mothers at Malipayon Village, Brgy. Tiza, Roxas City for our undergraduate thesis. In this connection, may we ask your permission to gather data for our study among child bearing mothers on this Barangay. I and my group mates namely Mary Grace Delid, Hazel Joy Clavero and Kessie Espinosa will personally conduct a survey using self-made questionnaires on the 16th day (Friday) to the 18th day (Sunday) of September 2011.
