EVALUATION At the end of the nursing care done to our client, we attained a clearer and deeper understanding about

the patients disease state. Furthermore, we were able to gain learning on the importance of the patients prescribed drugs on the management of his condition. We were not able to resolve the main problem of the existing condition but fortunately we have been successful in rendering nursing interventions to every situation wherein our help is needed such as maintaining airway patency and as well as monitoring of vital signs that plays significant role in identifying impending deterioration of patients condition. The whole exposure was not useless. The care was worth doing. At the end, we were able to enhance our nursing skills in the clinical setting specifically in the Emergency Room.

HEALTH TEACHINGS Medications: Instructed watcher to administer religiously prescribed maintenance medications. (Paki butang nalng sa mga tambal dre pati route ug time.. kai wala koi copy man gud.. ) Demonstrate first and then encourage the patient to perform passive Exercise: range of motion exercise to help to recover from body weakness and at the same time Range of motion exercises does not strain the body because either no weights or light weights are used. These stretches and exercises make most individuals feel healthier and better. Exercise increases the cellular glucose uptake by increasing the number of cell Treatment: receptors. Encouraged patient and instructed significant others to turn patient on sides every two hours to prevent bed sores. Fluid replacement and insulin administration intravenously (IV) are the primary and most critical initial treatments for diabetic ketoacidosis. Follow-up must be available with a health care practitioner. If patient is vomiting must be admitted to the hospital or urgent care Outpatient: center for further observation and treatment. Instruct the significant others and the patient to return for the follow up check up to monitor the condition of the client and for the continuity of care. This will also help prevent future complication by treating it earlier if Diet: ever symptoms occur. Since the client has diabetic ketacidosis the significant others were instructed to avoid giving food to the patient that are high in sodium such as (can goods, dried fish etc.)and high in fat foods like meat products and also foods which are stimulants such as( coffee, tea and soft drinks). Sweet foods like cakes, juices, chocolate, candies should be avoided to prevent increase in blood glucose levels beyond normal range. Instructed watcher also to watch for episodes of hypoglycemia by monitoring blood sugar thru HGT. During hypoglycemic states, instructed the watcher to feed patient with juice thru NGT and refer to the physician. The watcher

also was instructed to see further dietary orders by the doctors if NGT
