- 2x2 Picture - 3x5 Picture (whole body) - Transcript of Records - NSO Birth Certificate - Barangay clearance - NBI clearance - Mayors clearance - Municipal Court clearance - Regional Trial Court clearance - Municipal Judge clearance - Provincial PNP clearance - C-2 or R-2 clearance - Personal History Statement - Affidavit of Parents Consent (21 yrs old below) - Marriage Contract of Parents - Local Police clearance

2. PAFROPP applicants will be notified to report at PAFPMC for the Special Written Exam (SWE). Only those applicants who will submit the above documents will be allowed to take the SWE.
