Call Narrative Narrative: The child, Winona (age 4), has severe developmental delays. The child still drinks from a bottle, is unable to form sentences when speaking, and wears a diaper. The mother, Winona, has mental health issues and has done nothing to iddress her child's developmental issues. The mother has fled the State of Ohio after learning her child was being removed from her care. The mother has a history of hitting the child and being inappropriate with the child in the past. The role of the father, Paul, is unknown at this time.
Miscellaneous Information: The source gave 4lNMIMMMM> as an alternate telephone number. The mother left Ohio for Suffolk County before the child could be removed from her care. After arriving in Suffolk Cty, the mother was observed striking the child in the chest while at a Cty office waiting for housing assistance. The mother then fled Suffolk cty and went back to Ohio. Progress Note Narrative: case received narrative: WENDY, THE MOTHER OF 16 YR OLD AUTISTIC CHILD (TIMOTHY) WAQS FOUND IN A MOTEL IN BED, UNABLE TO MEET THE CHILD'S NEEDS. MOTHER IS EXTREMELY MALNOURISHED AND IN A BAD SHAPE. THERE IS NO FOOD IN THE RESIDENCE FOR BOTH MOTHER AND CHILD. THERE IS FECES ON THE WALL ABOVE THE CHILD'S BED. MOTHER BLED ON A PLATE AND HAS BEEN UNASBLE TO MOVE AROUND TO TAKE CARE OF HERSELF OR CHILD. (cAUSE OR SOURCE OF BLEEDING IS UNKNOWN AT THIS POINT). LAST NIGHT THE CHILD WAS SEEN RUNNING NAKED IN THE PARK UNATTENDED, TRYING TO GET INTO PEOPLE'S CARS AND KNOCKING ON PEOPLE'S DOORS. ON CALL WORKER VICKI SHUTTE CONDUCTED INITIAL CONTACT: SCR came in. SCW contacted the source. He reported that the family is up at the hospital. 8/13/04 HOSPITAL ViSIT: SCW spoke to the head nurse, Carol Loughran. She reported that the mother and child were at the hospital last Sunday. She reported that a bag was found on the woman with several different names. SCW explained to her that the child was found last night running through the park with nothing on and trying to get into peoples cars and windows. She reported that at this time we have no dea if he was hungry or not.
SCW went to the hospital and attempted to speak with Tim. He was extremely autistic. The mother is extremely ill. She reported that she has been in the area for about three weeks. She stated that she doesn't know where she came from. She reported that Tim's father died and that she has a short memory right now. She stated that she has no relatives up here? When asked if she had relatives in the city she said it was to far. SCW asked how she paid for the hotel? She reported that someone gave her the money. SCW went through family members and friends. She reported that she has no one. TELEPHONE CONTACT: SCW contacted Alicia. She reported that we have no home for the child and the must keep the child in hospital. SCW spoke with Doctor Cosby. He reported that the mother and child was here 2 days ago because the child was out of medication. The child was given more medication. The mother brought him in. It was explained that we do not have a home that we could keep Tim safe and met his needs. SCW reinsured lim that we would follow up Monday first thing to find out where we are going to place him. SCW and another staff member at the hospital went through the mother's bag. In the bag SCW found several names and items.
