Interested Principal Cardholders must fill-out and submit this Convert- to-Installment application form to EastWest Bank Credit Card Office through fax numbers 830-8900, 830-8911 or email at cards@eastwestbanker.com.

Cardholder has the option, subject to EastWest Banks approval, to convert new retail transactions consisting of local and foreign retail purchases, online purchases and mail-order transactions forming part of their latest statement of account (SOA) and/or the cardholders current unbilled retail transactions. Cash Advance transactions, monthly amortizations of other installment programs and transactions already forming part of the Cardholders revolving balance are not eligible for the Convert-to-Installment Facility. The minimum amount required for Installment conversion is Php5,000.00 per transaction and must be within the Cardholders available credit limit. All Convert-to-Installment applications must be received at least five (5) banking days prior to the Cardholders payment due date.

Upon approval of the Convert-to-Installment application, the Fixed Monthly Installment amount will be billed to the Cardholders EastWest Bank Credit Card account on his next and succeeding billing statements until the Convert-to-Installment availment is fully paid. e. Cardholder must allow at least five (5) working days for processing of Convert-to-Installment application. EastWest Bank has the sole discretion to approve or reject the Cardholders Convert-to-Installment application and, if such application is denied, EastWest Bank has no obligation to furnish the Cardholder a notice or reason of such rejection. EastWest Bank also has the sole discretion to approve only a portion of the installment amount applied for. The Cardholders EastWest Bank Credit Card account must be active and current. Should the Cardholders EastWest Bank Credit Card account be suspended, cancelled, or not renewed or should the Cardholder fail to make payments or otherwise breach or violate the Terms and Conditions Governing the Issuance and Use of EastWest Bank Credit Cards, all of the unpaid Convert-to-Installment availments as well as the entire outstanding balance in the Cardholders EastWest Bank Credit Card account shall immediately become due and demandable. In case of partial payment by the Cardholder of the installment amount, the unpaid balance shall incur the usual finance charges charged on revolving accounts in accordance with the Terms and Conditions Governing the Issuance and Use of EastWest Bank Credit Cards. Any and all finance charges already imposed upon the Cardholders total outstanding balance or any part thereof to be converted into installment, whether the same consists in the Cardholders previous balance and/or new purchases/transactions, will not be adjusted, refunded or credited back to the Cardholder upon approval of the Convert-to-Installment application. In signing this Convert-to-Installment Application Form, I understand and agree that: (1) it is based on my instructions and has no implication on my relationship with any establishment or card issuer; (2) I shall pay the fixed monthly installment indicated above for the whole payment tenor I chose which fixed monthly installment shall form part of the total statement balance on my EastWest Bank Credit Card; (3) I may opt to pay only the Minimum Payment Due on my total statement balance wherein only a percentage of my monthly installment is paid, in which case I will be imposed a finance charge in addition to the installment interest; (4) In case my EastWest Bank Credit Card is suspended, revoked or cancelled, all my unbilled monthly installments shall immediately become due and demandable and form part of my total outstanding balance; (5) In case of pretermination, I agree to pay an Installment Pre-termination Processing Fee equivalent to 5% of the remaining principal balance or Php500, whichever is higher; (6) I shall continue to be bound by and comply with the Terms and Conditions Governing the Issuance and Use of the EastWest Bank Credit Cards which are incorporated herein by reference and of which I was furnished copies by EastWest Bank as well as the Terms and Conditions and Promo Mechanics of this Convert-to-Installment Application.
