SCOPE

This document describes the General Safe Work Practices (GSWP) Procedure to be used by TSK and SUBCONTRACTORS during the preparation and construction of all process plants, general facilities, storage and handling facilities, utilities and other services. And for off plot activities and demobilisation relevant to construction of an Aromatics Plant producing Paraxylene and Benzene among other compounds, in the Shuaiba industrial area within the State of Kuwait. This GSWP Procedure is specific to the construction and demobilization phases of this SITE and TEMPORARY SITE. This TSK General Safe Work Practices (GSWP) Procedure shall encompass: TSK Consortium. All Subcontractors engaged on the KPPC SITE and TEMPORARY SITE Vendors and suppliers as defined within the work scope. OWNER. (As PMC when engaged in direct KPPC related activity.)

This General Safe Work Practices (GSWP) Procedure applies to: The SITE The TEMPORARY SITE.

This General Safe Work Practices (GSWP) Procedure shall be distributed to all organisations engaged in the works at SITE and TEMPORARY SITE.

VISITOR

A practice is a set of guidelines established to help workers perform a task which may not require a step-by-step procedure. A persons or group of persons, requiring entry to the SITE or TEMPORARY SITE, not normally engaged in any activity there-in, having not completed the MAJOR Induction and not holding a permanent identification pass. PURPOSE

The Purpose of the General Safe Work Practices (GSWP) Procedure is to establish safe work practices at the SITE and TEMPORARY SITE to minimize the risk of injury, illness and property damage. The General Safe Work Practices (GSWP) Procedure is a document designed to identify the types of work activity which may be covered in a specific procedure, however require an overview for practical day to day purposes, or may require some specialised information or training in order for craft personnel, to perform their work safely.

RESPONSIBILITIES

In support of the SITE and TEMPORARY SITE General Safe Work Practices (GSWP) Procedure the positions identified within section 5 shall have responsibility. 5.1 SITE MANAGER

The Site Manager shall display personal commitment to the EH&S Management System through obvious site actions related to the safety of the personnel on the project, and through support of the TCM EH& S Manager. 5.2

DEPUTY SITE MANAGER

The Deputy Site Manager shall display personal commitment to the EH&S Management System through obvious site actions related to the safety of the personnel on the project, and through support of the SK EH& S Manager. He Shall: Ensure that the requirements of the General Safe Work Practices (GSWP) Procedure are adhered to on SITE & TEMPORARY SITE. 5.3 SITE EH&S MANAGER

The SITE EH&S Manager shall: Shall ensure the implementation and adherence to this General Safe Work Practices (GSWP) Procedure and report violations and/or noncompliance issues to project management for immediate corrective action. Assist the Site Manager in the general supervision of the EH&S program. Audit and control of due implementation of the General Safe Work Practices (GSWP) Procedure Conduct frequent inspection on site, related to the General Safe Work Practices (GSWP) Procedure and direct appropriate corrective action. Prepare inspection reports for the Site Manager for his review and action. Ensure the requirements of the General Safe Work Practices (GSWP) Procedure are cascaded to vendors and subcontractors. TCM, SK & SUBCONTRACTORS EH&S MANAGERS & RELEVANT ORGANIZATIONS

Shall ensure that all employees adhere to the requirements of this General Safe Work Practices (GSWP) Procedure and take immediate corrective actions in case of unsafe situations noted during inspections. In particular subcontractors are to ensure their compliance with the General Safe Work Practices (GSWP) Procedure 5.5 CONSTRUCTION MANAGERS

The Construction Manager shall: Assist the SITE Manager in planning and implementing the EH&S Program Ensure that the TSK EH&S Plan is implemented at the SITE & TEMPORARY SITE. He shall ensure that the General Safe Work Practices (GSWP) Procedure is implemented at the SITE & TEMPORARY SITE. 5.6 SUBCONTRACTOR SUPERVISORS

The subcontractor Supervisors are responsible for ensuring that practices identified in the General Safe Work Practices (GSWP) Procedure are applied to personnel under their supervision.

CRAFT LABOR

Craft labor is responsible for compliance with work practices at SITE & TEMPORARY SITE as described within this General Safe Work Practices (GSWP) Procedure.

GENERAL SAFETY RULES FOR SITE & TEMPORARY SITE

In order for a health and safety program to be effective, it is vital that it be understood and implemented at all levels from management to all employees. 6.1 Report unsafe conditions to your immediate supervisor. Promptly report all accidents/injuries/incidents to your immediate supervisor. Use eye and face protection where there is danger from flying objects or particles, (such as when grinding, chipping, burning and welding, etc.) or from hazardous chemical splashes. Dress properly. Wear appropriate work clothes, gloves, and shoes or boots. Loose clothing and jewelry shall not be worn. Operate machines or other equipment only when all guards and safety devices are in place and in proper operating condition. Keep all equipment in safe working condition. Never use defective tools or equipment. Report any defective tools or equipment to immediate supervisor. Properly care for and be responsible for all personal protective equipment (PPE). Wear or use any such PPE when required. Lockout or tag out or disconnect power on any equipment or machines before any maintenance, unjamming, and adjustments are made. Do not leave materials in aisles, walkways, stairways, work areas, or other points of egress. Practice good housekeeping at all times. Training on equipment is required prior to unsupervised operation. FIRE PREVENTION All portable fire extinguishers will be conspicuously located, accessible and maintained in operating condition. Portable fire extinguishers will receive an annual service check and a monthly visual inspection. These will be documented on the tag on the extinguisher or other form. All employees must know the location of fire fighting equipment in the work area and have knowledge of its use and application. Only approved safety cans shall be used for handling or storing flammable liquids in quantity. Only the original container or a safety can will be used. When heat producing equipment is used, the work area must be kept clear of all fire hazards and all sources of potential fires will be eliminated. Fire extinguishers will be available at all times when utilizing heat-producing equipment. INDUSTRIAL HYGIENE AND OCCUPATIONAL HEALTH Employees exposed to harmful gases, fumes, dust, and similar airborne hazards will be furnished protection through proper ventilation or personal respiratory equipment.

SAFETY RULES FOR VISITORS

The use of mandatory Personal Protective Equipment (PPE) shall be necessary for all the Visitors requiring access to the SITE & TEMPORARY SITE. Compliance with the following general rules by all visitors, including business associates and guests, shall ensure a safe work environment for visitors. Admission to a workplace is conditional upon each visitor abiding by the following health and safety rules: All Visitors shall be required to attend the MINOR Visitors EH & S Induction prior to issue of a visitors badge. EH&S induction shall advise the visitors the details of the TSK Consortium expectations, the SITE & TEMPORARY SITE PPE requirement, SITE & TEMPORARY SITE Medical Facilities, Emergency alarms and Sirens and SITE & TEMPORARY SITE Assembly and evacuation procedure. SITE & TEMPORARY SITE required Personal Protective Equipment must be worn at all times while at a construction area. Restricted work areas may require the wearing of additional PPE like, hearing protection, Hand gloves etc. Where instructed to do so by TSK EH&S or other permanent personnel, the visitor shall follow specified safe work practices related any work being undertaken and the hazards present. A NO SMOKING POLICY is in effect and compliance is mandatory except within designated smoking areas. Individuals who are under the influence of alcohol or illegal drugs, or, who are otherwise impaired so as to pose a safety risk, shall be prohibited on the SITE & TEMPORARY SITE. The alcohol and drugs policy of the project shall be on display at the SITE and TEMPORARY SITE. Horseplay, fighting, harassment of any kind, and otherwise interfering with another person is strictly prohibited.

HOUSEKEEPING

The importance of good housekeeping at a workplace is paramount in the prevention of accidents and injuries. Good housekeeping shall be a vital element of the SITE & TEMPORARY SITE EH & S Program. Workplace housekeeping is traditionally defined as keeping the job site clean and orderly. This shall involve but shall not be restricted to: Maintaining floors and surfaces. Keeping access way, exits and stairs free of clutter, clearly marked and well-lit. Controlling minor spills and responding to them quickly when they occur. Properly using & maintaining of equipment and tools. Ensuring adequate and safe storage areas. Correct handling and disposing waste.

The benefits from adhering to these basic principals shall be: That slips and trip accidents are reduced because the areas are kept clean, in good condition and free of spills.

Fire hazards are reduced because materials are properly stored, combustible materials are not piling up. In SITE & TEMPORARY SITE offices, exits are not blocked. Back injuries are curbed because material handling is minimized. Routine housekeeping activities shall be incorporated into all regular work practices.

PERSONAL PROTECTIVE EQUIPMENT

In terms of hazard control, personal protective equipment (PPE) is considered a method of last resort and shall not be used as a substitute for other reasonable measures which would result in the control of a hazard. Personal protective equipment is the last line of defense a person has against a hazard that may be encountered on the job. The proper use of this equipment may reduce or eliminate the extent of harm or injury and therefore its importance must not be under-estimated. It is critical that the appropriate personal protective equipment for the situation is used, and that: 9.1 PPE limitations are fully explained and understood by all employees. PPE is properly fitted for the individual; It Shall be ensured that the person using the personal protective equipment is trained in its use, care and maintenance; That it is regarded by the person using it as normal attire for working in that environment or with the particular hazard. SKIN, HANDS AND BODY PROTECTION

Clothes are a major line of defense against hazards on the job. Employees shall always dress suitably for work. Cotton or Wool clothing with full sleeves and long trousers/ coverall shall be considered as appropriate clothing for the SITE & TEMPORARY SITE. Clothing made of synthetic fibers which is readily ignited by or melted by heat or electric flash shall not be encouraged on the SITE & TEMPORARY SITE. Special body apparel may be required to prevent contact with: Noxious gas, liquid, fumes or dust. Objects that may puncture cut or abrade the skin. A hot object, liquid, or molten metal. Radiant heat.

Gloves are effective against most minor cuts, scrapes and abrasions and are recommended when handling sharp or abrasive materials. Specialized personal protective equipment for hazards includes: Finger guards. Thimbles and cots. Hand pads, mitts and barrier creams.

Specialized PPE specified by Material Safety Data Sheets for hazardous chemicals indicate what shall be required for safe handling of that specific hazardous material or chemical. This shall be applicable before working with controlled products.

Apparel of an employee must be of a type and condition that will not expose him to unnecessary and avoidable hazards. Where there is danger of contact with moving parts of machinery; The clothing must fit closely around the body. Dangling neckwear, bracelets, wristwatches, rings, or like articles must not be worn. Cranial and facial hair must be completely confined or cut short. Employees whose duties are regularly performed in areas and under circumstances where they are exposed to the danger of moving vehicles shall be required to wear distinguishing apparel or devices of highly visible material. EYE AND FACE PROTECTION

9.2

Eye and face protection shall be designed to protect the worker from hazards such as: Flying objects and particles. Molten metals Splashing liquids. Ultraviolet, infrared and visible radiation (welding).

This type of equipment may be classified into two types. 1) The first type, basic eye protection includes safety spectacles with side shields, mono frame goggles and eyecup goggles (as with some styles of flame cutting and gas welding goggles). Clip-on side shields must be used if they are not a built in feature of the spectacles. Prescription lens may be acceptable safety eye wear only if the lens provider has certified them as safety eye wear. In these cases, side shields must be clipped in place. 2) The second type face protection, includes chemical and impact resistant (plastic) face shields; metal mesh face shields for hot, humid conditions and chainsaw use; and welders shields or helmets with specified cover and filter plates and lenses. Basic eye protection should be worn with (underneath) face shields. Comfort and fit are important in the selection of safety eye wear. Lens coatings, venting or fittings may be needed to prevent fogging or to fit over regular prescription eyeglasses. Safety glasses should be cleaned daily or more often if required. 9.3 HEAD PROTECTION

Safety headgear is designed to protect the head from the impact of flying and falling objects, bumps, splashes from chemicals or harmful substances, and contact with energized objects and equipment. Safety hats must be approved to standards. The Safety Hard Hats must: Provide protection from impact to the side and back of the head.

Where a hazard of electrical contact is present, the worker must use a safety hat which is specifically designed to provide protection from this type of hazard. Safety hats must be adjusted to fit securely on the head. Where there is a likelihood of the hat falling off, or being blown off, chin straps must be used. All parts of the headgear must be compatible and maintained in accordance with the manufacturers instructions. If attachments are used with the headgear, they must be specifically designed for use with the specific headgear issued. Safety hats should be inspected before and after each shift. Any signs of wear or damage to the suspension harness, indicates that it must be repaired or replaced. Similarly, any damage to the shell indicates it must be replaced. A visual inspection of the shell should look for breakage, cracks, discoloration, chalky appearance and brittleness. The entire safety hat must be discarded if it was subjected to any penetration or significant impact. Safety hats should be cleaned using warm water and mild soap. Solvents, such as paint thinner, should be avoided because it can damage the material. They should not be painted. They should be stored in a clean, dry location. Although it does not have to be worn while a worker is inside a vehicle. Safety hats must be worn by traffic control persons at all times while on duty. FOOT PROTECTION

Safety is designed to protect where there is danger of injury to feet through falling or moving objects, or from burning, scalding, cutting, puncturing, slipping or similar causes. The following types of hazards require particular protective features which are incorporated into the footwear design: Safety footwear should always be laced up and securely tied to prevent a tripping hazard or the footwear from falling off. It must be maintained in good condition. exposure for sound levels averaging 88 dB (A) is 4 hours; for 91 dB (A), 2 hours; and for 94dB (A), 1 hour. Similarly an unprotected person may be safely exposed to noise levels of an average of 82 dB (A) over 16 hours and 80 dB (A) over 24 hours. Where practical, it is preferable to reduce the level of noise at, or close to its source through the application of engineering controls. Administrative controls, such as limiting the amount of time an individual worker is exposed excessive noise through job rotation, should also be considered. The use of hearing protection is the method of last resort, but must be used where engineering and administrative controls are not feasible. Hearing protection is available in three general types: 1. Disposable ear plugs (made of pliable material, one size fits all but can only be used once). 2. Permanent plugs (must be fitted to provide a good seal but can be washed and reused); and 3. Ear muffs (when properly fitted and worn, generally provide more protection than plugs). 9.6 RESPIRATORY PROTECTION

Personnel are sometimes exposed to respiratory hazards generated by equipment, materials, or procedures such as spray-painting and welding. Although proper work practices and engineering controls may be used to reduce these hazards, often the only practical control is respiratory protective equipment. Protection is ensured not only by the respirator but also by its proper selection and use. To select the proper respirator for a particular job, the worker must know the characteristics of the hazard, the anticipated exposure, and the limitations of the equipment. Respiratory equipment should only be selected by someone who understands all three factors. Respiratory hazards may be present as: 1. Gas: Common toxic gases in construction are carbon monoxide and hydrogen sulphide. The SITE poses hazards of Chlorine and Ammonia gas. 2. Vapors: Vapors are produced by solvents such as xylene, toluene, and mineral spirits used in paints, coatings and degreasers. 3. Fumes: Welding fume is the most common type of fume in construction. Other examples include pitch fume from coal tar in built-up roofing and from diesel engines. 4. Mists: The spraying of paints, form oils and other materials generates mists of varying composition. 9.6.1 Dust: Dust is generated by crushing, grinding, sanding or cutting. Two common dusts in construction are fibrous dust from insulation materials and non-fibrous silica dust from sandblasting. 9.6.2 Controls Work areas must be ventilated to reduce hazards from dust, fumes, mists, gases or vapors. Where ventilation is not practical, workers must be provided with respirators appropriate to the hazard and be trained to use and maintain the respirators properly.

9.6.3 Respiratory protection falls into two major categories: A. The first is Air Purifying Respirators (APRs) which have particle (dust) chemical cartridges but no visor plate. The air is inhaled from the surrounding air but cannot replenish or increase its oxygen content. B. The second category is Atmosphere Supply Respirator, which includes self-contained breathing apparatus (SCBA), airline systems and protective suits that completely enclose the worker and incorporate a life support system. 9.7 FALL ARREST/RESTRAINT SYSTEMS

Full Body Harnesses are used to provide workers working at heights above ground a level of freedom to move and protection from falls. The SITE & TEMPORARY SITE Fall Protection policy requires that, where a person is exposed to the hazard of falling from a work area that is: 2 meters or more above the nearest safe surface or water; above a surface or thing that could cause injury to the person on contact; and above an open tank, pit or vat containing hazardous materials,

The person shall wear a fall arrest system. A guardrail, personnel safety net or temporary flooring may be used instead of a fall arrest system. Fall-arrest system shall consist of a combination of the following. Full body harness. Lanyard. Rope grabs. Lifeline. Lifeline anchor. Connectors.

All full body harness and lanyards must be approved by TSK. Full body harnesses must be snugfitting and worn with all hardware and straps intact and properly fastened. 9.7.1 Maintenance The following is only a guideline for maintenance, care and storage. All equipment used in a fall arrest system must be maintained to manufacturers specifications. Any equipment in need of maintenance must be tagged and removed from service. All hardware should be cleaned and lubricated with light oil. Store in a clean, dry location free of corrosives and harmful fumes. Store out of direct sun light. Clean synthetic webbing with a wet sponge. Use a mild detergent for more difficult stains. Equipment should always be dried thoroughly after becoming wet. Keep away from excessive heat. Lubricate parts as recommended by manufacturer.

9.7.2 Inspection Fall Protection equipment must be inspected by the user before each and after every use. A detailed inspection must also be conducted at regular intervals, (minimum yearly), by a competent person. Check shall be made for the following during inspections: Webbing: Examine all webbing on both sides from end to end. Flex webbing over fingers bending it to expose any signs of damage. Look for evidence of damage related to cuts, tears, abrasions, heat burns, kinks, knots, broken strands or excessive wear. Discolored, fused, brittle or melted fibers may indicate signs of damage from heat, paints solvent or chemicals. Lanyards: should be examined for signs of shock loading. Any piece of equipment that has arrested a fall must be removed from service, destroyed or returned to manufacturer for evaluation. Hardware: Evidence of defects or damage to hardware elements will require the equipment to be taken out of service. Cracks, sharp edges, deformation, corrosion, chemical attack, excessive aging or excessive wear should be examined. Check the metal wear at the base of the D-ring and make sure the D ring pivots freely. Check buckles to ensure that they are not bent or distorted and that they can move freely back and forth and engage correctly. All locking snaps and karabiners should operate smoothly. The latch (keeper) must close securely against the snap nose. Rivets: Make sure rivets are holding tightly and have not pulled through the webbing. Rivets should not be bent. Pitted rivets indicate chemical damage. Stitching: Make sure there are no more than two breaks in the thread or any stitch pattern. Grommets: Grommets must be tight, not distorted or broken. Check for corrosion, dents, sharp edges or cracks. Ropes and cables: Examine the rope from end to end, rotating it as the inspection proceeds. Rope must be free of knots and of consistent diameter. Look for discoloration, broken, cut, crushed, worn, or deformed fibers. Damage can be caused by chemicals, welding, painting and exposure to light or heat. Retractable lines: Check the wire rope life line for broken strands. The locking mechanism should be examined at different extended lengths to ensure correct operation. The hook should be secure on the line, in good condition and free to swivel. Check indicator button, where applicable, to ensure the unit has not arrested a fall.

TRAINING

Training and education cannot be over-emphasized as a means of learning a healthful and safe approach to employee work effort. Knowledge of the safety rules and how and when to function under the rules, supplemented by compliance, is essential to safety. Employees scheduled for any safety and health training will attend such training. New employees will be provided orientation training and will be furnished information and literature covering the company health and safety policies, rules, and procedures. This orientation training must be provided prior to the employee's exposure to the work environment.

Individual job/task training will be provided to all employees. Included in this training is: the applicable regulations/standards for their job; the recognition, avoidance, and prevention of unsafe conditions; areas and activities that require personal protection equipment; and how to use protective equipment (such as respirators, etc.). On-going safety training sessions will be conducted to provide information and training on new equipment, new procedures, new chemicals, refresher/remedial and training in specific areas. Such training may be held in conjunction with the safety briefings/meetings held as a result of PTW and STARRT Procedures. Various individual Workers' Safety programs specify that training be provided to specific employees such as: o Fire extinguisher training o Confined space entry o Respirator care and use o Hazard communication o Lockout/tag out procedures o Industrial truck/forklift operation o Electrical work. Training addressed above will be documented in the employees' personnel record

HAZARD IDENTIFICATION, ASSESSMENT, AND CONTROL

Hazard identification and elimination is not only an inherent responsibility of supervision in providing a safe workplace for employees, but also requires employee involvement. As such, hazard evaluation and control shall be an on-going concern for all. It is the responsibility of everyone (management, supervisors and all employees) to identify, report, and correct, all possible hazards. Employees are particularly important in this process as they are in the best position to identify hazards in the workplace and day-to-day operations. Reporting hazards is a protected activity and no action will be taken against anyone for identifying unsafe conditions. Reports should be made to the EH&S Manager or supervisor for appropriate action. TSK has a procedure for conducting inspections of workplaces/jobsites for compliance with health and safety rules. The purpose of the in-house inspection is to identify hazards and unsafe practices before they cause an injury or accident. After completing jobsite or facility inspections, the person making the inspection will:

Safe work practices are ways of controlling hazards and doing jobs with a minimum of risk to people and property. Safe work practices are general in nature and can be applied to many different situations (i.e. use of ladders, hand power tools, etc.). The SITE & TEMPORARY SITE recognizes that certain jobs or tasks may be hazardous or critical and therefore require more specific, step-by-step procedures to ensure work is carried out safely, but do not require the application of a Permit To Work. Each General Safe Work Practice must be developed to fit the particular workplace, equipment and process. These Safe Work Practices shall be developed over a period of time. The Safe work practice shall Identify the hazards associated with the situation and describe the relevant steps to be followed in order to have the work carried out safely. May use the format found in Appendix 1 If a Safe Work Practice is developed, they shall be numbered consecutively. Safe work Practices shall provide standards for how work is carried out. Supervision who actually execute the task should be involved in the development of Safe work Practices. EXAMPLE OF SAFE WORK PRACTICE.

Before any work or maintenance is performed on any machine, equipment, tool, or electrical system, they will be made totally safe before work starts by removing any source of energy or power to them, such as electrical, air/hydraulic pressure, spring/stored energy, or thermal (heat/cold). The Lockout/Tag out Procedure provides for a safe method of working on, near, or in machinery or equipment that can cause serious injury. This program will be used by all employees to ensure that the machine or equipment is stopped, isolated from all potentially hazardous energy sources, and locked out before employees perform any servicing or maintenance where the unexpected energization or start-up of the machine or equipment, or release of stored energy, could cause injury. ELECTRICAL

Live electrical parts shall be guarded against accidental contact by cabinets, enclosures, location, or guarding. Open circuit breaker openings or knock out holes, broken receptacles/switches, missing covering plates, etc., will be reported to supervisors for repair or replacement. Working and clear space around electric equipment and distribution boxes will be kept clear and assessable. Circuit breakers, switch boxes, etc. will be legibly marked to indicate its purpose. All extension cords and electric powered tools (except double insulated) will be grounded. Ground prongs will not be removed. Electric cords and their strain relief devices will be in good condition, with no splices.

All flywheels, shafting, pulleys, belts, gears, sprockets, chains, and fan blades will be guarded/enclosed when located below seven feet above the floor or work platform. Guards installed on machinery and equipment, such as air compressors, conveyors, drill presses, etc., will not removed when operating. Guards removed for servicing or other work on the machine or equipment will be immediately replaced upon completion of the work. Woodworking equipment, such as power saws, radial arm saws, or table saws, nor portable abrasive grinders, will not be operated unless all required guards are in place. Featherboards and pushboards will be used when necessary. COMPRESSED GAS CYLINDERS

12.6

All gas cylinders shall have their contents clearly marked on the outside of each cylinder. Cylinders must be transported, stored, and secured in an upright position. They will never be left laying on the ground or floor, nor used as rollers or supports. Cylinder valves must be protected with caps and closed when not in use. Oxygen cylinders and fittings will be kept away from oil or grease. Oxygen cylinders will be stored distant from any fuel gas cylinder, or separated by a fire barrier at least five feet high. When cylinders are hoisted, they will be secured in a cradle, sling-board, or pallet. Valve protection caps will not be used for lifting cylinders from one vertical level to another. LADDERS

12.7

Ladders will be inspected frequently to identify any unsafe conditions. Those ladders which have developed defects will be removed from service, and repaired or replaced. They will be tagged or marked as such. Portable ladders will be placed as to prevent slipping, or if used on other than stable, level, and dry surfaces, will be tied off or held. A simple rule for setting up a ladder at the proper angle is to place the base from the vertical wall equal to one-fourth the working length of the ladder. Portable ladders will extend at least 1 meter above the upper level to which the ladder is used to gain access. The top of a stepladder will not be used as step. Only one person will be on a ladder at a time. FLAMMABLE AND COMBUSTIBLE LIQUIDS

Only approved safety cans, original containers, or portable tanks will be used to store flammable or combustible liquids.

WELDING AND BRAZING

Combustible material will be cleared from the area around cutting or welding operations. If the combustible material cannot be cleared or the work cannot be moved, then the welding/cutting will not be done. Welding helmets and goggles will be worn for eye protection and to prevent flash burns. Eye protection will be worn to guard against slag while chipping, grinding and dressing of welds. Welding screens will be used and in proper position to protect nearby workers from welding rays. Cables, leads, hoses, and connections will be placed so that there is no fire or tripping hazards. Cables will not be wrapped around the welder's body. Oxygen cylinders will be stored distant from fuel gas cylinders, or separated by a noncombustible fire wall with a one-half hour rating at least five high. Valve protection caps will be in place on cylinders not in use. Ventilation is a prerequisite for welding in any confined spaces.

12.10 TOOLS Hand tools with broken/cracked handles, mushroomed heads, or other defects will not be used. Files will have handles installed. Take special precautions when using power tools. Defective tools will be removed form service. Power tools will be turned off and motion stopped before setting tool down. Tools will be disconnected from power source before changing drills, blades or bits, or attempting repair or adjustment. Never leave a running tool unattended. Power saws, table saws, and radial arm saws will have operational blade guards installed and used. Anti-kickback teeth and spreaders will be used when rip sawing. Portable abrasive side-winder grinders will have guards installed covering the upper and back portions of the abrasive wheel. Wheel speed ratings will never be less than the grinder RPM speed. Air compressor receivers will be drained frequently to prevent buildup of water in the tank. Compressed air will not be used for cleaning purposes except when pressure is reduced to less than 30 psi by regulating or use of a safety nozzle, and then only with effective chip guarding and proper personal protective equipment. Any employee-furnished tools of any kind are not permitted.

12.11 SAFETY RAILINGS AND OTHER FALL PROTECTION All open sided floors and platforms 2 meters or more above adjacent floor/ground level will be guarded by a standard railing (top and mid rail, toe board if required). All stairways of four or more risers will be guarded by a handrail, or stair rails on the open side. Handrails or stair rails will be provided on both sides if the stairs are more than 1.5 meters wide. When a hole or floor opening is created during a work activity, a cover or a barricade must be installed immediately. Safety harnesses, belts, lanyards, lines, and lifelines may be used in lieu of other fall protection systems to provide the required fall protection. Adjustment of lanyards must provide for a not more than a 2 meter fall, and all tie off points must be at least waist high.

12.12 SCAFFOLDS Scaffold platforms more than 2 meters above the ground, floor, or lower level will have standard guardrails (consisting of top rail, mid rail, and toe board) installed on all open sides and ends of platforms . Planking will be laid tight, overlap at least 250 mm wide, and extend over end supports 200 mm. Mobile scaffolds will be erected no more than a maximum height of four times their minimum base dimension. Scaffolds will not be overloaded beyond their design loadings.

12.13 FORKLIFTS Only authorized and trained employees will operate forklifts and other industrial trucks. Safe speeds, load handling, turning, and other safe driving practices will be followed at all times. Operators will ensure loads are stable and/or secure before moving. Employees will not operate any forklift that is in need of repairs, defective, or unsafe. Such forklifts will be removed from service for repair.

12.14 AERIAL LIFTS Only trained employees will operate aerial lifts (cherry pickers, extensible and articulating boom platforms). A body belt or harness will be worn and a lanyard attached to the boom or basket when working from an aerial lift. Employees will stand firmly on the basket floor, and not sit/stand/ climb on the edge of the basket. Planks, ladders, or other devices will not be used for additional elevation. Aerial lift trucks will not be moved with workers in an elevated basket. Lift controls will be tested each day prior to use.

12.15 EXCAVATIONS AND TRENCHES Any excavation or trench five feet or more in depth (or less than five feet and showing potential of cave-in) will be provided cave-in protection through shoring, sloping, benching, or the use of trench shields. Specific requirements of each system are dependent upon the soil classification as determined by a competent person. A competent person will inspect each excavation/trench daily prior to start of work, after every rainstorm or other hazard increasing occurrence, and as needed throughout the shift. Means of egress will be provided in trenches four feet or more in depth so as to minimise lateral travel for each employee in the trench. Spoil piles and other equipment will be kept at least 1 meter from the edge of the trench or excavation.

An Example of Safe work Practices is included in the Appendix 2. Additional safe work procedures shall be developed in accordance with recommendations from SITE & TEMPORARY SITE EH&S&S meetings and as well as indications from job hazard analysis (JHA) conducted by supervisors.

A: Compliance with SITE & TEMPORARY SITE EH &S Standards

All abrasive blasting operations shall be in full compliance with the C.S.A. Standard Z180.1-00 regarding supplied breathing air for the blaster. Ambient air systems shall be installed, maintained and operated with accepted supplied air respirators in accordance with the respective manufacturers instructions. Supplied breathing air systems shall have a valid air purity certificate from an accredited laboratory. The ambient air system air intake assembly shall be located in an area that is contaminant-free. The air intake shall be: 1. Located in an area free from potential sources of contaminants. Care shall be taken in selecting a location, especially in the proximity of local exhaust ventilation outlets, equipment/automobile/truck exhausts, adjacent industrial activity, etc. 2. located outside with the inlet 1-3 meters above grade; and 3. Equipped with a wind direction indicator.

Users and those required to maintain ambient air systems shall be knowledgeable about: 1. Where to locate air inlets 2. replacement schedule for filters; 3. method for determining air flow to the respirator user; 4. method for adjusting the flow rate on pressure output; and 5. Method, frequency and location for the collection of compressed breathing air for analysis.

The user shall read, understand and be knowledgeable about instruction manual and shall operate the air pump in accordance with the instructions. The user shall inspect the inlet muffler and hose assembly and the air line before each shift and report any defects found to the supervisor. The user shall keep a log of the number of hours the air pump operates. The inlet muffler shall be changed every 500 hours and the outlet filter every 200 hours or more frequently if odors or defects are observed. The ambient air pump will supply a maximum of 10 CFM of breathing air to one user or a maximum of 15 psi. The pressure relief valve shall not be adjusted to exceed these maximums. Before starting work, the user shall allow the air to purge through the respirator and hose assembly for a few minutes to eliminate any odors or tastes in the respirator assembly. The respirator shall be donned and the outlet pressure gauge re-checked prior to entering the blasting work area. When finished working, the user shall exit work area wearing the respirator with the air still flowing. Once outside the work area, the respirator shall be removed, the pump turned off and the air supply hose disconnected, using the quick disconnect couplers. The air pump and related accessories shall be stored in a clean, dry area. In the event of discovering any problems while using the air pump, the user shall refer to the instruction manual and consult with the supervisor.

C. The Airline Respirator

The user shall read, understand and be knowledgeable about the instruction manual and shall use the respirator in accordance with the instructions. The blaster shall use abrasive blasting material which does not contain silica. A carbon monoxide warning indicator shall be affixed to the inside of the helmet. Any indications of carbon monoxide on the indicator will be reported to the supervisor and the matter will be further investigated and corrected. The blaster shall use hearing protection and eye protection inside the helmet. The user shall inspect all components of the respirator system daily for signs of wear, tear or damage that might reduce the degree of protection originally provided. All damaged or excessively worn components will be immediately replaced. The respirator shall be cleaned and stored in accordance with the instructions in the user manual.
D. Personal Protective Equipment

The blaster shall wear an appropriate suit approved for blasting, as well as
substantial gloves. The blaster shall wear substantial safety footwear.
