AZTA MEMBERSHIP QUESTIONNAIRE I. IDENTIFICATION 1. Please list your name, street address, phone number, and e-mail address.

2. Where do you teach? Please include your center or temple website if you have one. II. AUTHORIZATION 1. 2. 3. 4. What is your school (e.g., Soto, Rinzai, Soen, Thien...) and within that school what is your dharma lineage and who should we contact about your credentials? Who authorized you to teach and when? Are you sanctioned as a full teacher? If not, what restrictions have been placed on your teaching and what circumstance would lift these restrictions? What sort of interviews are you authorized to give? (e.g., koan/kong-an interviews, Soto dokusan, etc.)

If yes, where was this (multiple places may be listed), what was expected of you, and for how long were you a resident? Did you train as a non-resident? What sort of positions did you hold at the places you practiced before you became a teacher? Roughly how many days of intensive retreat have you sat when you were not the teacher? Does your practice experience include long (over a week) retreats and/or solo retreats? If so, please describe. Who was/were your main teacher/s? With whom did you complete your training? In the tradition in which you have been sanctioned, were you required to resolve koans as part of your formal? If so, was there a specific koan/kong-an curriculum? And, if so, what as it? .Who could confirm that you completed this curriculum as required?

Are you authorized to sanction others to teach within your lineage? If so, at what level? Can you sanction others to fully transmit your lineage and to eventually fully transmit it themselves? What is your typical retreat schedule, and how often do you lead retreats?
