Elimination Pattern

His normal defecation pattern is twice or thrice daily. The color is described as yellowish to brownish in color, no foul smelling odor and with semi formed consistency. His normal urination pattern is 2-3 times daily with no difficulty. The color is yellowish- slightly brownish with light faulty odor but no presence of blood being noted. Sleep is a basic human need. It is a universal biological process common to all people. We require sleep for many reasons: to cope up with daily stresses to prevent fatigue, to conserve energy, to restore the mind and body and to enjoy life fully. Sleep enhances daytime functioning. Sleep and Rest Client usually sleeps Pattern for 5-7 hours and usually takes naps in the afternoon for an hour. His sleeping environment is described with good ventilation and sleeps with the lights off/on. He doesnt use any medications to get him sleep. He doesnt experience problems waking up at nights and no history of insomnia. There are no rituals before going to bed.

He has a total of 45 hours of sleep with episodes of waking up in the middle of the night due to interruptions of sleep. Sleeping environment is also good to sleep and rest because the client was confined in a private room, and has an aircondition as a form of ventilation.

CognitivePerceptual Pattern

The client is able to read and write and solves simple mathematics. He finish his college degree. The patient doesnt have any difficulty both in hearing and vision. The client doesnt use hearing aid nor eye glasses. The client can speaks coherently and answers questions.

Client is able to speak and communicate. The patient was able to repeat whispered words. Glasgow Coma Scale scored as 15.

It also represents a progression of mental abilities from simple to complex, illogical to logical. According to Piaget, cognitive development is an orderly, sequential process in which a variety of new stimuli must exist before intellectual abilities can develop. (Fundamentals of Nursing 8th ed, by Kozier and Erbs, pp 356, 981) Clients cognitive ability has not been affected. Self-concept is ones mental image of oneself. A positive self-concept is essential to a persons mental and physical health. Individuals with a positive self-concept are better able to develop and maintain interpersonal relationships and resist psychological and physical illness. An individual possessing a

strong self-concept should be better able to accept or adapt to changes that may occur over the life span. How one views one-self affects ones interaction with others. (Fundamentals of Nursing 8th ed, by Kozier and Erbs, pp 1003

RoleRelationship pattern

He is a father of 1 child and he is married. Today he lives in one of their house in Antipolo City he has been a good provider to the needs of his family. He is friendly, good communicator and very sociable to his neighbors. He is also a law-abiding citizen.

His relationship with the members of the family deepens probably because of the concern and love for him. His loneliness in the hospital had been relieved and alleviated because some of his friends visited him.

A role is a set of expectations of about how the person occupying one position behaves. Role performance relates what a person in particular does to the behavior expected of that role. There was passing of role as a head of the family, it starts when he got paralyzed.

SexualityReproductive Pattern

The client is comfortable with the gender he belongs. In the past 40 years of life, he doesnt have any mind confusion when it comes to her sexual preference. Sexual health is an individual and constantly changing phenomenon falling within the wide range of human sexual thoughts, feelings, needs, desires. For most people, sexual health is not considered until its absence or an impairment is noticed. A positive sexual self-concept enable people to form intimate relationships throughout life. Spirituality generally involves a belief in a relationship with some higher power creative force, divine being, or infinite source of energy. Ones spirit titer is influenced by numerous factors such as life experiences, coping skills, social supports and individual belief systems. Value-Belief Pattern

He is a Roman Catholic, he believes in only one God. He do attend mass regularly together with his family he doesnt fail to pray every night. h He believes that everything happens for a reason and only God controls everything as he always says. They believe also in some superstitious beliefs. He is a God-fearing person and that what he gives emphasis to his child.

The clients spiritual beliefs were more strengthened because he believes that God is the only one who can save him. He always pray for his condition and to his family.
