old ROOM NO: WARD1- BED 20 EMPRESSION/DIAGNOSIS: G1P1 pregnancy uterine PHYSICIAN: Dr. Princess Lorezo cephalic term live birth delivered via NSVD PATIENTS OUTCOME CRITERIA As soon as the patient is discharge from the hospital, the patient will be able to acquire basic knowledge, develop skills and positive attitude on the care of the postpartum patient, specifically patient will be able to: ASSESSMENT Assess for any signs and symptoms of infection. Instruct patient to immediately go to the nearest hospital if signs of infection occur such as: redness, tenderness in the breast and pain, drainage. Assess whether the patient was able to understand the health teaching that was given the student nurse. Encourage client to do proper hygiene to prevent from contamination to microorganisms that will cause infection. Remind patient to carefully watched what she will do everyday NURSING ORDER

Assess patients knowledge regarding the present health condition.

Assess client to see whether she understands the importance of proper hygiene. Assess clients usual daily activity pattern. PLANNING 1. Follow a regular check up with the physician 2. Take the medication as indicated at the right time, right route and right dose

Encourage to have a regular consultation for continuous care. Instruct patient to take medication at prescribed time, dosage and route.

Assess the wound healing.

IMPLEMENTATION: M Comply her take home medication that was given by her physician. E Provide a clean, germ-free environment.

Remind the patient and the SO to schedule a return visit for monitoring until the wound healed. Encourage patient and SO to follow drug therapy. Instruct SO and patient to maintain clean environment and facilitate relaxation at feeding time. Discuss on to have a consistent recovery of the patient, promote healing process and to avoid infection. Advice patients family to let the patient do hand washing to prevent spread of microorganisms. For continued care, remind patient that she must participate in the follow-up schedule.

T- Observe proper perineal care, hotsitz bath and take medications regularly.

H Wash following hands before and after breastfeeding and always clean the breast before breastfeeding. O Remindpatient to comply her out patient referrals or follow up check-up.

Adviced patient to eat nutritious food D- Adhere to the dietary regimen as prescribed. specially foods that are rich in fiber, for wound healing. S Allocate time for reflection, praying and reading bible. EVALUATION 1. Evaluate the importance of having planned scheduled of treatment and check-ups. 2. Evaluate patients response or selfcompliance. Discuss to the patient the importance of compliance and regularity of check-up and continuity of care and treatment. Identify patients reaction and determine the level compliance of the patient and possible factors that would hinder him from complying with the treatment plan. Encourage spiritual guidance.

Client describes her daily routine and identifies adjustment that she can predict will need to be made.
