ON WHITEBOARD: Simplify process of seeing patient, record plan for ourselves and others, remind ourselves of important info, give plan esp for evenings and weekends, think through diff dx and tests, collect important results in one place, provide a 24 hr summary, provide info to nurses and others about patient, show people we actually saw the patient (protect us from lawsuits), make writing a discharge summary easier, simplify/organize thoughts for our presentations, etc.

Show the junior how to think through a problem; List a differential diagnosis; Free our time for teaching (junior can organize patient info for us); Understand how the juniors thinking; Make a good clear progress note which communicates how youre thinking to the junior

After pt 1: What can you learn from the note about the student? Can picture the patient without actually seeing him. Even if I dont know the patient, I should be able to look at your A/P and understand the important points about the care of your patient Make it easy for the next doctor or nurse - Ceftri (10/14), use if...then, can when statements

This note is on your handout which you can review later. The clinical observation is important because it shows the student what you think is important to note. But because of time, I want to focus on the A&P. Do you have to write a note like this every day NO! But you do want at least 1. Notice Assessment. problem list in order of importance, differential diagnosis with plan anyone should be able to pick up this chart and know what were doing with the patient.

Remember - even if we havent read the H&P, we should be able to look at your A/P and understand their condition and important points regarding their care. Think about a differential diagnosis.
