RULES AND REGULATIONS GOVERNING USE OF CHURCH VEHICLE

Tripping Policies and Procedures Church Vehicle Requirements: 1. Valid drivers license. 2. Driver will use the vehicle only for Church or other covered entity business within the Scope of the drivers approved conditions. 3. Wear his/her safety belt (when applicable). 4. Observe all traffic laws. 5. Unauthorized person will not be allowed to drive vehicle. 6. Consumption of alcoholic beverages or narcotics prior to the operation of a vehicle is prohibited. 7. Driver will ensure that windows, headlights, and tail lights are clean and operational at all times. 8. Turn signals must be utilized by driver at all times when making a turn or changing lanes. 9. Do not tail-gate 10. Where vision is obstructed, do not make turn-abouts, i.e. (u-turn). 11. Vehicles are not to be parked in such a manner as to constitute a traffic hazard to the driving public. 12. Designated speed limits on streets and highways will be observed. 13. MUST BE 25 YEARS OF AGE. A complete list of all passengers including the driver must be provided to the church office or placed behind the driver side visor on the van. This is in case of an accident and identification must be made of passengers. Maintenance Requirements: Before operating a Church or covered entity vehicle, the driver is responsible for checking to see that all necessary safety equipment and control devices are in good condition before he/she operates it. For vehicles used to transport people, when equipped, such as an activity van, the following items, in addition to those listed above, should be included: 1. Reflectors 2. Fire extinguisher, Type B-C 3. First Aid Kit

VEHICLE POLICIES I. Purpose A. Church vehicle shall be limited to organized groups and activities related to the ministry of CGS Church. B. Vehicle shall not be borrowed for personal use (except for the Senior Pastors). They shall not be loaned or leased outside the church without written request presented to the church Trustees one month in advance. C. Passenger limitation on church van will be 15. II. Trustees Committee A. B. C. D. Responsible for maintaining adequate rules and regulations for the vehicle operation. Responsible for adequate insurance & license. Must authorize all repairs/or replacements beyond normal service, except for emergency repairs. A member of the trustees shall be assigned responsibility for: 1. Procedure for scheduling vehicle operation. See paragraph II, VI and V below. 2. Maintenance check made first of each month. 3. Maintain file of vehicle reports in the Church Office. 4. Keep up-to-date list of APPROVED DRIVERS that are 25 years of age or older without any significant traffic violations. All approved drivers must be member of CGS Church. This list shall be maintained in the Church office. 5. Vehicle is registered in the name of CGS Church and is in the custody of the Trustees. E. Vehicle keys and emergency equipment bag: 1. Vehicle Keys: Van Key is kept in the Church Office. MAKING OF DUPLICATE KEYS BY ANYONE IS STRICTLY FORBIDDEN. If a key is lost, contact the Church Office. 2. Emergency Equipment Bag: For safety purposes, an emergency equipment bag is located under the drivers seat of the vehicle. It contains a fire extinguisher, first aid kit and reflective triangles. III. Passengers for Vehicle Operation (including driver): A. A maximum of 15 passengers, including driver for the van. B. Passengers are responsible for their personal possession on all trips. Driver and/or group leaders to assist passengers. IV. Requests for Vehicles A. RESERVATIONS: The vehicle may be reserved through the Church Office with the church Secretary on a first come, first served basis. Any exceptions to the rule must be made in writing to the Church Trustees. KEYS: It is the responsibility of the driver to make arrangements for obtaining the vehicle keys during normal office hours, Monday Friday, 9:00 am 3:00 pm and Saturday & Sunday, 9:00am 12:00pm. CANCELLATIONS: Reservations must be canceled with two weeks notice from the reservation date. Failure to cancel within the appropriate time may result the forfeiture of future vehicle use. Exceptions to the cancellation clause should be discussed with the Church Secretary or Chairman of the Church Trustees. B. CHURCH CALENDAR: The Church Calendar, which includes all vehicle reservations, is maintained in the Church Office IF it is questionable whether a group is qualified to use the vehicle, contact the Chairman of the Church Trustees. C. OTHER: The Church Trustees are responsible for Vehicle usage, and may restrict any group that is found to be abusive and negligent in its use.

V. Driver Rules A. Qualified drivers must be approved according to Section II.D.4 above B. All traffic violations are the sole responsibility of the driver and the program person designated as in charge of the trip event. C. Driver shall be responsible for making all reports required. 1. A VEHICLE REPORT is to be completed by the driver and returned to the Church Office when the vehicle is returned. The vehicle will be checked after use and any discrepancies from these policies shall be directed to the driver and/or program person responsible. 2. Gasoline must be purchased by, and at the expense of each group. Upon return, fuel in the fuel tank must be full. Failure to do so may result in forfeiture of future use. 3. If a servicing point is designated, the vehicle is to be taken for the checking of oil, water and tires, otherwise driver must do at nearby station before leaving on the trip. 4. Upon completion of the trip, driver will fill the fuel tank, and return the vehicle to designated parking area. Vehicle must be returned with interior CLEAN, and ALL REFUSE REMOVED. The keys should be returned to the church office. 5. Complete Vehicle Report; LOCK vehicle before returning key. 6. Driver and/or group leader have full authority for keeping order and discipline in the vehicle at all times. 7. In the event of an accident, the program leader and driver shall be responsible for filing appropriate police reports, and reporting any accident to the insurance company as quickly as possible. 8. A copy of these rules and regulations to be furnished to all drivers, a copy retained with Vehicle Reports in the church office, and a copy to all members of the Church Trustees. VI. Safety: A. An Emergency Equipment Bag containing a fire extinguisher, first aid kit and reflective triangles is located in back of van properly secured. B. There will be no smoking or use of alcohol/drugs in the vehicle at any time. C. Driver/passengers must keep seat belts fastened while traveling. VII. CONTINUED OPERATION OF/OR DISPOSITION OF CHURCH VEHICLE IS CONTINGENT UPON THE DECISION OF THE CHURCH TRUSTEES. THE CHURCH TRUSTEES RESERVE THE RIGHT TO AMEND THESE REGULATIONS AT ANY TIME. VII. Church Vehicle Maintenance RESPONSIBILITY OF THE CHURCH TRUSTEES PURPOSE: A. To provide assistance to the Chairman of/and the Church Trustees in supervising the use of the church vehicle in accordance with the Church Vehicle Operating Procedure, Rules and Regulations as approved by the Church Trustees. B. To maintain vehicle reports and other records as may be necessary. C. Check weekly to determine that: 1. Vehicle returned to designated parking area with interior clean and all refuse removed by last group using the vehicle. 2. Vehicle is not damaged in interior and exterior by last user. 3. Motor oil is at the proper level, all tires are inflated properly, and water levels are checked in radiator. A member of the Trustees will be selected to perform the duties as outlined in this sub-paragraph.

4. Vehicle is lubricated at proper time, oil changed as required by Operator Manual and exterior washed and interior cleaned, if need. A member of the Church Trustees will be selected to perform the duties as outlined in this sub-paragraph. D. Report to Chairman, Church Trustees, any infraction to vehicle rules and regulations. E. Report to the Church Trustees at each meeting regarding the Vehicle usage, scheduling and any recommendations to improve the vehicle operation in the ministry at CGS Church. ------------

USE OF CHURCH VEHICLES Statement of Policy: A. Vehicle owned and operated by CGS Church is for the purpose and support of ministry. It is made available to ministry groups and outreaches of CGS Church to provide safe and dependable group transportation for Churchrelated activities and ministry functions. B. Vehicle owned by CGS Church will not be leased, for profit or otherwise to outside groups under any circumstances C. This policy applies to vehicle leased by the church and its members for the purpose of a church sanctioned (approved, allowed) ministry or function. In this situation, members are to decline (refuse, reject) the leased vehicle insurance. Exclusion to this policy is leasing through a contracted and licensed transportation firm that provides its own drivers and insurance. D. Church vehicle may be used for events sponsored by organizations outside the church upon approval by the Church Trustees or Church Administrator. All approved organizations using the Church Vehicle are required to abide by this policy. E. This policy applies to the use of churchs vehicle the purpose of transportation for church ministry. Procedures: 1.0 GENERAL GUIDELINES FOR US 1.01 General Use Use of Church vehicle shall be in compliance with these guidelines, the policies of our insurance carrier. Failure to abide by any of these rules shall result in the denial of future use of Church-owned vehicle. 1.02 Vehicle Use Church vehicles are to be used exclusively for the support of ministries and church programs. Any use apart from this purpose is strictly prohibited. 1.03 Vehicle Capacity Church van will be used for no less than five and no more fifteen passengers. SPECIAL NOTE: Recent studies by the National Highway Traffic Safety Administration (NHTSA) shows that the rollover rates for 15-passenger vans loaded with more than 10 occupants were nearly three times the rate of those that were lightly loaded. The report does not claim the vans are defective or poorly constructed but simply warns that, when fully loaded, the vans center of gravity shifts rearward and upward, increasing the likelihood of a rollover. The shift in the center of gravity will also increase the potential for loss of control in panic maneuvers. 4

1.04 Approved Drivers Only a Church approved driver may operate the vehicle. A driver must have a valid drivers license. A record of the drivers license must be maintained in the church office. A driver must be capable of operating the assigned vehicle and performing emergency procedures. SPECIAL NOTE: To obtain approved driver status; all drivers will have a Motor Vehicle Records (MVR) conducted by the Churchs Administrator or Church Trustee. 1.05 Driver Responsibilities The driver will be responsible for making certain that all procedural rules are followed, pre and post inspections are done and recorded, and financial obligations paid. The driver will be responsible for exhibiting proper driving behaviors and following all laws during operation of the Church vehicles. The drivers are responsible for the safety and security of all passengers in the vehicle(s). 1.06 Vehicle Scheduling Keys are to be checked in and out from the church office. Requests for vehicle use must be completed through the calendaring process and approved by the Church Administrator or Church Trustee. 1.07 Vehicle Pick-up/Drop-off The driver will have the responsibility of picking up the vehicle from the designated area and returning it to that same area after the trip. 1.08 Refueling and Cleaning The vehicle must be filled with gasoline at the trip and returned to the storage area with the tank filled. The vehicle must be cleaned at completion of use/trip including removing all trash and another items. The interior and exterior shall be cleaned and washed if needed prior to returning the vehicle to the designated area. 1.09 Refueling Direct Charges or Reimbursement Authorization Groups with authorization to spend against the Church budget OR with approval from the church office may charge gasoline at CGS Church or can be reimbursed for out-of-pocket gasoline purchases. When gasoline is charged to the church account, the driver must submit a receipt with a completed purchased/funds request form to the Church Account within three days of returning the vehicle to the Church.

1.10 Refueling Direct Charges or Reimbursement without Authorization Groups without authorization to spend from the transportation line item in the annual budget OR prior approval from the church office must pay for gasoline individually. 1.11 Seat Belt Use Each passenger must have access to and wear a seat. It is the drivers responsibility to ensure all persons in the vehicle are secured with a seat belt at all times.

1.12 Age Limitations Children under age are not allowed to be transported in the 15-passenger van. Exceptions to this procedure must be approved by the Church leadership. An example of an exception is transporting congregation members of all ages from off-site parking for outdoor and worship services. 1.13 Supervision and Safe Sanctuary Adequate adult supervision (at least two legal adults) is required on all trips where minors are involved. It is the responsibility of the trip director to secure adequate adult supervision and adhere to the Safe Child Policy adopted by the Church. 1.14 Financial Obligation due to Violations CGS Church will not be responsible for financial obligations due to the actions of drivers operating Church vehicle in violation of local ordinances. 1.15 No Smoking Statement Smoking is not permitted in a Church vehicle. 1.16 Cell Phone Use Use of a cell phone, by the driver, is only permitted either while the vehicle is parked or when used by a passenger who is not driving. 1.17 Insurance Coverage Insurance coverage on Church vehicle will meet or exceed all government requirements and will be reviewed annually. The Church will carry liability insurance to cover collision (accident) on the vehicle. These rules and regulations governing the use of church vehicles shall be promulgated and effective immediately this 6th day of January 2012 and copies to be distributed to all local pastors and CORE.
