1. Please read the instructions carefully given in this form and the Brochure before filling this form. 2. Read the eligibility criteria and medical fitness requirements carefully and ensure that you meet the requirements. In case of doubt, write an email or contact the nearest DGS approved training institute conducting GP rating course. 3. Information on career prospects, pattern of examination, and related aspects are given in the brochure that can be downloaded from the website www.seafarers.edu.in 4. If this form is being printed from the soft copy please use good quality printers and set the printer to A4. Poorly printed forms can be rejected or the candidate may miss the deadline due to delays in communication and resubmission of form. 5. Pages 1 to 4 can be printed back to back, but page 5 and 6 must be on the single sheet (Physical Endurance Test). Send pages 3 to 6, together with the DD and a self addressed. Complete the table of attachments on page 4. 6. Application forms must reach the CET Office latest by 24th February 2012. 7. Examination will held be on Sunday 18th March 2012, at 28 centers listed inside. (choose any three) 8. Eligibility Criteria: It is the responsibility of the candidate to ensure eligibility for admission. (Candidates not satisfying the eligibility criteria will be informed by 1st March 2012. Incomplete form will be considered as not eligible. (read more details in the brochure).
Minimum - 10th Pass (SSC)% (Recognized State Board) Aggregate In case marks for English at 10th or 10+2 are less than 40%, min 40%, an min 40 % in English, Pass in Maths & Science then Marks in Degree or Diploma need to be min. 40% Age: at the time of admission to the institute 1 July or 1 January, as the case may be : Not less than 17 yrs and 6 months and not more than 25 years at the time of entry to the course. Height and Weight (Height without shoes: weight in indoor Medical Fitness: See more details in the brochure No physical disability, No loss of speech or impediments clothing) Minimum Distance Vision (Unaided) 1.0 (6/6) in better eye and (6/9) in other Height 157 cm. (5'2") eye. Normal colour Vision (To be tested by Ishiara test chart) Weight 48kg (105 lbs.) Before joining an institute candidates will be required to undergo medical examination by DGS approved Doctor. A list of DGS approved doctor is provided on DG Shipping website, as well as on BES website.

9. The Common Entrance Test will be English only. Written paper 1.5 hrs.; followed by Physical Endurance Test; and an oral test (English conversation and reading) 10. For the sake of speed the preferred mode of communication will be email; SMS; and telephone. 11. Address of each examination center will be made available on the website by 1st March 2012. 12. Admit Card / Hall ticket will be sent by speed post at least 15 days before the examination using self addressed envelope provided by the candidates. Candidate will receive BES code number and roll number through email/SMS/ and it will also be displayed on the website. In case a candidate does not receive admit card in time, he/she must carry a photo copy of the application form, photo identity with date of birth, and copy of the message from BES to the examination center as a proof of being a registered applicant. No candidate will be allowed to take the examination without a BES code number and roll number. 13. Examination fee is non-refundable in any circumstances. (General Rs. 800/; for ST/SC/Female Rs. 400/) 14. Results will be declared within six weeks after the date of examination. Candidates will be informed by email/SMS. Result will also be displayed on the website. 15. Candidates will be allotted institutes on the basis of given choices, merit score, and priority for ITI and ward of Seaman. Reserve/waiting list will be declared in case a candidate does not get an institute within the given choices, or the seats become vacant due to candidates failing to join or pay the fees or due other reasons. 16. Candidates will be required to report to the allotted institute within one week of allotment, and will be expected to pay full fee to the institute within the time given by the institute. No request for change of institute will be entertained once the institute is allotted. Candidates failing to report or pay within the given period will risk losing their seat and initial deposit. 17. Candidates must possess a Passport or apply for one at least one month before the start of the course. 18. Award of marks for the test will be checked by a Supervisor. All care will be taken in verifying entries before declaring the results. There will be no provision for appeal or reassessment of papers. 19. Legal Jurisdiction: All disputes related to Common Entrance Test and allotment of candidates to the institutes will be subject to the jurisdiction of Bombay High Court only.

BEST has no branches or agents. Candidates are advised not to pay any fee in advance to the any institute or agent, and not fall in a trap of promises of any kind.

Note: For General Purpose Ratings: Eyesight - There should not be any degree of squint or any morbid condition of either eye or the eyelids, pressure of trachome and iris complications sequela. Candidates must possess good binocular vision (fusion faculty and full field of vision in both eyes). Movement of the eyeballs must be full in all directions and the pupils should react normally to light and accommodation. Distance Vision (Unaided) 1.0 (Values given in Snellen Decimal Notation.)
