Instructions for filling Online Application

Online submission of application may be made at website http://174.133.46.146/Jipmer2012/frmMainFrame.aspx. Detailed instructions are available at the site. Candidate should read the instructions carefully before making any entry or selecting options. Candidate should supply all the required details while filling up the online form. Mandatory fields are marked with * (asterisk) sign. The filling of online application contains two parts: MD/MS Entrance Exam 2012 Part I New Application (Closes on 23.11.2011) Part II Complete Application (Closes on 03-01-2011 at 5:00 PM)

1. In Part I New Application, candidate will have to fill basic information. On submission of details, candidate will be prompted to check the details and make correction, if any, in the application. 2. When Next button is pressed, and after selecting the payment mode a page with Reference No will be generated. Please note down reference number, user name and password or take a print out of the page. The application is incomplete without part II Complete Application. Part II Complete Application contains filing procedure of payment details, Candidates who can pay application fee online though payment gateway or through DD 3. Those who want to pay online through net banking can go directly to part II Complete Application after completion of part I New Application. Candidate will have to supply reference number, user name, password and Name to continue to Part II Complete Application. 4. Take the Print out of the application 5. Keep the Xerox copy of the application after filling all the details for any future reference. 6. DD should be in the name of "Accounts officer, JIPMER payable at PUDUCHERRY, Union Territory" incase of DD payment. 7. Failure to post this printout duly signed with the Bank DD or NEFT/SBI Payment details, copies of OBC/SC/ST/OPH certificates will lead to rejection of the application 8. Please Speed Post to Professor (Academic), JIPMER, PUDUCHERRY - 605 006. 9. Application should necessarily be accompanied with an attested copy of the OBC / SC / ST / OPH certificate (wherever applicable). The certificate should have been issued by the Competent Authority. 10. Application without the OBC / SC / ST / OPH certificate would be considered only under the General Category. 11. Change in category after submission of application would not be considered under any circumstances. 12. No communication would be entertained in this regard. 13. Click New Application Enter the code as shown in image and click Submit Enter User Name, Password, Confirm Password, Email, Contact Number and click Register Enter Personal Details and click Next Select a Payment Mode and click Continue Note down the Reference Number, User Name , Password and Name Click Have a Print to print the application details Click Complete Application to submit the DD Details Enter User Name, Password, Online Application Number, Name and click Get Application Enter DD No, Date Of Payment ,Bank , Place and Click Submit Take the printout of the complete application form
