(MS WARD) Duties for 7 -3 shift 1. Receives endorsement and make rounds to assess patients condition, IVs, procedure and treatment to be done and will be performed.

2. Check the charts for new orders then classify according to attending physician. 3. Gives good nursing care to patients according to their needs which includes the following : a. Giving of medications and treatments to patients. a. Check if medicines are available, if no medicine is available at the patients box, make necessary requisition at the pharmacy if patient is a PHIC and is still not excess. b. In the case wherein the patient is already excess and a non PHIC, the nurse is responsible to prescribe the medicine to the patient or relatives. b.) Carrying out of doctors order. c.) Assisting doctors in some procedure as in minor operations, removal of stitches, dressings of wounds, cord dressings and NGT insertions. a.) Encourage and assist relatives of patients in giving baths to patient to keep patient clean and free from infection. e.) Supervise the midwives and attendants in the procedures allowed to be done by them and check results if satisfactory. f.) Does operate some medical gadgets that were hooked to the patient like suction machines, cardiac monitor, etc. g.) Responsible in nourishing patients on NGT. 4. Makes frequent rounds to all patients, thus assessing their condition as well thus referring any unusualities to the attending physician via the resident on duty. 5. Tactful in dealing with doctors, patients or relatives and family. 6. Give health teachings to patients and watchers. 7. Receives admission of patients from E.R for which the following are the duties and responsibilities : a. Supervises and helps midwives/attendants prepare empty bed for admission. b. Checks if IVF stand and oxygen tank with gauge is ready if patient to be admitted need it. c. Read the doctors order when chart and patient is received @ the ward. d. Checks the medication sheet for the medicines given. e. Carry out doctors order :

Transcribe doctors order to medicine ticket. Fill-in Kardex. Notify dietary of patients diet.

8. Communicates with dietary or dietary personnel regarding patients diet, noting changes if there is any and with janitors and attendants for the cleanliness of the ward. 9. Accompany doctors during their rounds then check charts for discharged patient and summary of services before bringing to the pharmacy for counterchecking of the medicine and supplies taken or used. 10. Carry out first those stat orders then for discharged patients prepare and give prescription for take home medicine and instruct patient or relatives regarding home medicine and follow-ups. Remove discharged patient from diet list. 11. Helps meet the spiritual needs of patients by informing pastoral sister in charge to call a priest if an emergency arises. 12. Makes prompt report of apparatus, equipment, instruments, etc that needs repair. 13. Supports hospital economy through avoiding unnecessary use of supplies, electricity and water. Additional duties for 3-11 shift 1. Gathers charts and check IVF for follow-up, request Resident on Duty to order for IVF to follow that would last until morning. 2. For excess and non-meds, make sure to provide prescription of prescribed medicine and IVF enough until morning dose to patient or watcher. 3. Distributes or checks if patient for stool and urine examination have specimen bottle. Teach patient how to save specimen for laboratory examination. 4. Instructs patient who will undergo test, treatment and operation on nothing per orem (NPO) after midnight or as doctors order. 5. If assigned @ the 1st floor wherein the delivery room is located, assist in the delivery room cases. 6. Endorsed to night shift everything done and to be done.

Additional duties for 11-7 shift 1. Re-instructs patient that will undergo test, treatment and operation on nothing per orem (NPO) after midnight or as doctors order. 2. Checks if data in the charts are complete. 3. Together with the midwives/attendants, re-enforce the collection of specimen for laboratory examination. 4. Checks if all patients for blood chemistry examination have been extracted with blood specimen.

5. For O.R. patients: Supervises midwives/attendants in cleansing enema, shaving and encouraging patient to take a bath afterwards then provide patient with hospital gown. 6. Pre-O.R teaching instituted such as removal of dentures, jewelries, nail polish and any foreign objects before entering the operating room, 7. Encourages patient and watcher to fix and tidy things thus providing them fresh and clean linens. 8. Endorses to A.M shift everything done @ night shift and what need to be done.

DUTIES AND RESPONSIBILITIES (ER) 1. 2. 3. 4. Receives endorsement from outgoing ER nurse. Make inventories on medicines and equipments at e-cart. Do housekeeping. Checks the medicine cabinet in the Emergency Room if the necessary emergency medicines are available for the next 24 hrs. 5. Checks availability of instruments and supplies needed for dressings and minor surgeries like suturing and debridement. 6. Checks if oxygen tank is full and working. 7. Check nebulizers and suction machines. 8. For newly admitted patient, carry out doctors orders at once. 9. Starts venoclysis to client per doctors order. 10. Start first doses of ordered meds including skin testing. 11. Assess patients vital signs before endorsing to ward/room of choice. 12. Send laboratory requests at the laboratory and instruct patient for proper specimen collection and NPO for those who will undergo blood chem. 13. Assists Resident-on-duty upon performing their rounds, minor operations and/or reviving patient during crisis situations. 14. Collaborates immediately to Pharmacy section for ordered meds. 15. Apply necessary interventions needed to stabilize patient's condition. 16. Endorses patient to ward nurse including the meds administered, procedures performed , lab exams and his condition. 17.
