Rules & Conditions: 1. The Yokohama Season Signature Offer only applies to retail sales of new replacement tires. 2. You must purchase individual sets of 4 tires (no singles or doubles) between March 15, 2012 to May 31, 2012 to be eligible for the offer. 3. To receive your rebate you must fully complete this original claim form, attach the original dated invoice and mail to Yokohama by June 22, 2012. 4. The invoice from a Canadian retail dealer must clearly identify the tire dealers name, invoice number, date of purchase, product purchased (including tire size) and quantity purchased. 5. Your Yokohama rebate cheque will be mailed to your address within 68 weeks upon receipt of a valid completed claim form. The cheque will be mailed to Canadian addresses only. 6. Available at participating Yokohama Tire dealers in Canada only. 7. May not be combined with any other Yokohama promotion or program. 8. Failure to provide original documents, incomplete documentation or illegible documentation will result in the return of your claim, unpaid. 9. Please ensure that you keep a copy of all forms including your receipt for your records. We will not be returning any proof of purchase submitted with this offer. 10. Yokohama Tire (Canada) Inc. is not responsible for lost, stolen, damaged or misdirected mail/cheques/forms. 11. All fields must be completed or your submission will be returned.
