Was the purpose of the charter explained to you by the project manager? Was a charter completed for the project? Was it reviewed and approved by you? Was the scope well defined and well documented in the charter? Was it the scope that was delivered? If not, were the changes well communicated and documented? Were there risks that were identified and included in the charter during the initiation phase of the project? If additional risks were identified during the remainder of the project, were they communicated and well documented? Were the following project roles identified in the charter?
