Incident in brief Root causes of incident On 28th Aug 11, at around 03.30pm The cause of incident was The deceased (Mr.Binu kumar investigated & found as Kashyap / Helper, aged about 22 years of M/s Sunil Singh ) who was 1) Violation of Work engaged for assisting electrical instructions given by Technician (Mr.Amarjeet Prasad & Electrical in charge. Remedial measures In order to prevent the recurrence of such incidents in future, the following measures are recommended, 1) Looking towards the said incident, Strict supervision is required based on criticality location of job. 2) SOPs (both Hindi & English for all routine & Non routine activity shall be prepared & all the concerned shall be educated 3) Risk assessment to be done on site for every job, explained to all the persons involved in the job (on daily basis till the job is completed) & also displayed at site. 4) While performing such type of jobs minimum safe distance to be maintained from the bare live line ( 132 kv line, A minimum 1.4 mtrs. distance to be maintained for shifting the ladder etc.)

5) Ladder handles to be insulated or FRP ladder (Fiberglass Reinforced Plastic) material as it has got advantage over aluminum ladder may be explored. 6) Specific Refreshment training to all the electricians to be rendered immediately. 7) The available aluminum ladder to be locked & kept in safe custody. After Risk assessment at the Work area, Ladder can be given to the concern team.

After touching the ladder to 132 KV line immediately Breaker got Tripped on protection relay and plant got tripped,which was reduced the severity of the incident.
