WHITEHALL DOGGIE DAY AND PET PARADE SATURDAY, SEPTEMBER 29TH, 2012 INDIVIDUAL HOLD HARMLESS AGREEMENT/WAIVER OF LIABILITY

To participate in the Whitehall Doggie Day and Pet Parade this Hold Harmless Agreement/Waiver of Liability must be signed by each individual with a Dog attending the event. Complete and mail this Hold Harmless Agreement/Waiver of Liability or bring it with you on the day of the event to give to an event official (only if you are unable to mail them to us in advance). No one will be allowed to participate without this form being submitted prior to the start of the parade. I/We assume all risk of bodily injury, property damage and personal injury that I/we may incur in participating in the Whitehall Doggie Day and Pet Parade event and I/we, for myself/ourselves, my child/our children, my/our heirs, executors and administrators do hereby, forever waive and release any and all claims against and agree to indemnify and hold harmless The Whitehall Chamber of Commerce, The Whitehall Parkway, The Township of Whitehall and all of their respective officers, employees, agents, representatives, successors or assigns of any kinds to be collectively referred to as Released Parties from any and all claims which may be made for any cause whatsoever arising as a result of or in connection with my/our my dogs participation in the Whitehall Doggie Day and Pet Parade. Acknowledgement of Risk: Owner is aware of the present and continuing inherent risks of injury, death, and property damage to Owner, Dog, and persons and dogs of some third party that are involved, and those not involved, in the event, including without limitation risks due to dog bite or infectious disease. Owner is voluntarily engaged in the event as an activity with knowledge of the known risks and other risks that may result from Dogs participation in the event, including but not limited to injury, death, or property damage from disease, stray dogs, dogs running away, other dogs in at event, other animals, or injury, death, or property damage caused by Dog to other dogs, animals or persons. Assumption of Risk: If Dog causes property damage, or bites or injures any dog, animal or person, including but not limited to the Released Parties, during or after the term of this Agreement, Owner agrees to assume full responsibility and liability for any injury, death or property damage, and Owner agrees to pay all resulting losses and damages suffered or incurred, and to defend, indemnify, and hold harmless the Released Parties from any and all resulting claims, demands, lawsuits, losses, costs of expenses, including attorneys fees. Release of Liability: Owner releases Released Parties from all liability to the Owner, and Owners representatives, guardians, successors, assigns, heirs, children, and next of kin for all liability, claims, damage, or demands for personal injury, death, or property damage, to the Owner or to the Owners dog or dogs, arising from or related to this Agreement or to participation in event, whether the injury, death, or property damage occurs on or off the training site.
