MECHANISM OF ACTION

DRUG STUDY INDICATIONS AND CONTRAINDICATIONS Indications: This medication is a short-term treatment of active duodenal ulcer:Maintenance therapy for duodenal ulcer patients at reduced dosage after healing of acute ulcers, The treatment of pathological hypersecretory conditions Contraindications:

ADVERSE REACTION/ SIDE EFFECT Adverse Reaction: CNS: Rarely, malaise, dizziness, somnolence, insomnia, and vertigo. GI: Constipation, diarrhea, nausea/vomiting, abdominal discomfort/ pain, and rare reports of pancreatitis.

NURSING INTERVENTION Observe 11 rights in giving medication Observe patient for Potential
toxicity results from decreased clearance (elimination) and therefore prolonged action; greatest in the older adult patients or those with hepatic or renal dysfunction. Watch out for Be alert for early signs of hepatotoxicity (though low and thought to be a hypersensitivity reaction): jaundice (dark urine, pruritus, yellow sclera and skin), elevated transaminases (especially ALT) and LDH. Inform the patient that even if symptomatic relief is provided by ranitidine, this should not be interpreted as absence of gastric malignancy. with other H2blockers, rare reports of arrhythmias such as tachycardia, bradycardia, atrioventricular block, and premature ventricular beats. Musculoskeletal: Rare reports of arthralgias and myalgias. Hematologic: Reversible decrease in WBC count.

Ranitidine is
contraindicated in patients known to have hypersensitivity to ranitidine bismuth citrate or any of its ingredients.

Integumentary:
Rash, including rare cases of erythema multiforme. Rare cases of alopecia and vasculitis.

DRUG STUDY INDICATIONS AND CONTRAINDICATIONS Indications: This medication is a prophylaxis and treatment of acute attacks of gout. Contraindications: Contraindicated in patients with blood dyscrasias.

11 rights in giving medication Advise patient to avoid intake of alcohol or foods high in purine such as sardines and anchovy paste. Advise patient to increase oral fluid intake to enhance excretion. Instruct patient to take med. with meals.

Avoid activities that may cause bruising or injury. (Colchicine may reduce the number of clotforming cells (platelets) in blood.) Avoid contact

with people who have colds or infections. It may lower the ability of the body to fight infection.

Inhibits the enzyme responsible for the conversion of purines to uric acid, thus reducing the production of uric acid with a decrease in serum and sometimes in urinary uric acid levels, relieving the signs and symptoms of gout.

Management of signs and symptoms of primary and secondary gout. Management of pt.s malignancies that result in elevation of serum & uric acid Use for patients with recurrent Calcium Oxalate Calculi whose daily uric acid exceeds 800mg/day(male) 750mg/day(female CONTRAINDICATION Contraindicated with allergy to allopurinol,blood dyscrasias and clients with hemochromatosis

CNS: Headache, drowsiness,peripheral neuropathy, neuritis, paresthesias GI: Nausea, Vomiting, Diarrhea, abdominal pain, gastritis, hepatomegaly, hyperbilirubinemia GU: Exacerbation of gout and renal calculi, renal failure HEMATOLOGIC: Anemia, Leukopenia, thrombocytopenia, bone marrow depression

Observe the 11
rights of administering drugs. Assess patient for any allergic reactions towards drug, discontinue on the first sign of rash. Encourage patient to drink 2.5-3L of water to decrease the risk of renal stone development. Report any unusualities like fever, chills and bruising. Monitor nutritional status: discourage organ meat, sardines,

salmon, legumes (high purine), gravies, alcohol Advise patient to take med. after meals.

Dosage: 1 tab Frequency: TID Route: P.O.

Provides elemental iron, an essential component in the formation of hemoglobin.

Indications: Iron deficiency Contraindications: Contraindicated in patients with hemosiderosis, primary hemochromatosis, hemolytic anemia (unless patient also has iron deficiency anemia), peptic ulceration, ulcerative colitis, or regional enteritis and in those in those receiving repeated blood transfutions. Use cautiously

GI: nausea,
epigastric pain, vomiting, constipation, black stools, diarrhea, anorexia. Other: temporarily stained teeth from liquid forms.

Observe 11 rights in giving medication Tell patient to take tablets with juice (preferably orange juice) or water, but not with milk or antacids to enhance its absorption. Instruct patient not to crush or chew extended-release forms to maintain its effectivity. Alert: Inform parents that as few as 3 to 4 tablets can cause serious iron poisoning in children. Caution patient

on long-term basis.
