UNIVERSITY OF THE CORDILLERAS COLLEGE OF NURSING 1st Semester 2012-2013

LEVEL II GUIDELINES/MECHANICS FOR CASE PRESENTATION

1. Upon the start of clinical rotation, the group should start looking for a case for their case presentation.
2. Before the end of the rotation, if the group decided to get the case for their case

presentation, all data & information should be taken/copied to avoid the reason of going back again to the hospital just to copy some missing information/data (some hospital administrations do not allow students to retrieve files of patients).
3. The students may seek the advice of their clinical instructor in choosing a case. 4. All cases should be taken only in the wards and in the community where the

students are assigned.


5. Cases to be taken should be in line with the concepts discussed in their NCM101

and CHN subjects


6. The chosen patient must be handled by the students for 3 consecutive days. 7. Students must write a letter addressed to the Dean, Area Head and Clinical

Coordinator. The letter should indicate the following: a. name and final diagnosis of the patient b. rationale why the case was chosen c. area of assignment where the case was obtained d. date of exposure when the case was obtained
8. The clinical instructor who supervised the students in the area shall be the

approving CI who signs the letter of the students.


9. The letter is then forwarded to the Clinical Coordinator/Area Head. The Clinical

Coordinator/Area Head takes note of the cases, makes sure that there will be no duplication of cases. The letter should be submitted on or before Sept 12,2012.

SUBMISSION OF CASE PRESENTATION OUTPUT

1. Submit 3 copies (1 copy shall be the receiving copy) of written report/output to the Clinical
Coordinator/Area Head from Sept.21-22 ,2012 during office hours only. FAILURE TO SUBMIT ON THE DATES STATED ABOVE WOULD BE AUTOMATIC 20 POINTS DEDUCTION FROM YOUR TOTAL RAW SCORE. 2. Written reports/outputs should be placed in a long brown envelope; properly labeled and signed. ex. CASE PRESENTATION OUTPUT

3. The photocopy of the approved letter must be included. 4. Written reports/outputs should be submitted personally by the group leaders at the Nursing Office from 8:00 am to 5:00 pm. only

5. No student or faculty member is allowed to retrieve the submitted output for the purpose of editing
and/or changing of case.

6. Case pres instructors should get their copy at the Nursing Office.
DURING CASE PRESENTATION

1. Always start the presentation by a short prayer to be led by the leader of the presenting group. 2. The case presentation should be limited to 2 hours (45 minutes presentation and 1 hour 15 minutes
question and answer portion).

3. Report at least 15 minutes before the said activity. 4. Be in your type A community uniform (no colored jacket) and proper haircut. 5. The members of the group must have their name tags (should be in large print) and I.D.
6. Leader introduces the members of the presenting group. 7. The assigned faculty summarizes the whole presentation. CASE PRESENTATION OUTPUT

a. The format of the pathophysiology/explanation of the problem is at the discretion of the


students on how they can properly and efficiently explain the case of the patient. They can use narrative and/or diagram (concept map). b. On the format of the NCP: Nursing Problem Explanation of the Problem/Pathophysiolog y Objective Nursing Intervention s Rationale Evaluation

NOTE: Use EXPECTED OUTCOME in the evaluation column if the NCP was prepared but was not implemented because the nursing problem was not encountered during the exposure.

c. The nursing interventions in the implemented nursing care plan (NCP) should be written in
PAST TENSE and the last column should be EVALUATION. However, for unimplemented NCPs, nursing interventions should remain in the PRESENT TENSES and the last column should be EXPECTED OUTCOME.

SUBMISSION OF FINAL CASE PRESENTATION OUTPUT


1. Be sure to get the corrected output from the instructor BEFORE LEAVING the case presentation room. 2. Submit the FINAL OUTPUT (revised output) to the Clinical Coordinator/Area Head (ONE COPY ONLY, SOFT BOUND, GREEN COLOR). 3. The original letter ( Letter of Request for the Case Presentation) should be included in the final output. 4. The revised output together with the corrected output must be placed in a long brown envelope properly labeled and signed.
