New Delhi 110001 No. BDM/31-1/2012 To Shri Pawan Rawal, Manager (LPG-S), Delhi Area Office, Indian Oil Corporation Limited, 2nd Floor, World Trade Centre, Babar Road, New Delhi - 110001 Sub: Proposal for Address Verification of customers of IOCL Ref: Your Office letter No. DAO/LPG/General dated 8-2-2012, email dated 2-3-2012 Dear Sir, Your proposal for facilitating address verification process in the Delhi Region through India Post has been examined and considered at this end. I am directed to inform you that your aforesaid proposal has been approved by Chief Postmaster General, Delhi Circle under the following terms & conditions: (i) The Customers verification forms (bilingual) duly printed will be handed over to SSPOs, Delhi East Dn., Krishna Nagar, Delhi - 110051, (designated as the Nodal Officer for this project) by IOCL in the hard copy as per Annexure 'A'. The DoP will be responsible for getting the Columns of the Verification form filled in from the customer/informer to the extent as may be possible for the surveyor/verifier (DoP staff) & also as required by the contents of the Form. The name of the customer, customer's registered address, Customer No. etc. on the form will be printed by IOCL. The survey will be done by the Postman/PRI(P) of the concerned Area PO. The charges for each Address/customer survey/verification will be Rs. 55/- + applicable service taxes, Edn Cess etc. The bill for Address verification will be raised by the Nodal Officer directly to IOCL. -2(vii) IOCL will pay 80% of the total estimated amount of the no. of customers' to be surveyed/verified through Cheque in favour of Sr. Postmaster, Krishna Nagar HO, Delhi - 110051. The rest 20% would have to be paid by IOCL on completion of the verification within 7 days on receipt of bill from the Nodal Officer. The DoP will not be liable for any incorrect/false information provided by the Customer/informer to the verifier/surveyor. Further, DoP will not be liable for any consequential losses/damages to IOCL arising out of such incorrect/false information in the verification form. The survey/verification job of 6000 Forms will be initially undertaken by DOP and may be completed within one month of receipt of forms by the Nodal officer. However, DOP shall not be responsible for delay ,if occurred in completion of work due to unforeseen circumstances beyond its control or for any adverse eventuality arising out during the verification process by the DOP staff. If in any court case/suit , preferred by a Customer, DOP is made a party, the IOCL will be liable to indemnify for the loss, if any to DOP. You are requested to confirm your acceptance.

One Area Manager Marketing from C.O. will also be deputed to assist the staff at the Division Office/Post Offices concerned.

Proposed procedure/guidelines for verification work

Customers verification will be assigned immediately after receipt of a hard copy of the details of customer in the format mutually agreed, i.e. the particulars of the customers details. 2. The SSPOs, Delhi East Dn., Krishna Nagar, Delhi - 110051 has been designated as the Nodal Officer for this project by the Competent Authority. The Nodal Officer will be monitoring the project and coordinate with IOCL authorities for carrying out the customers 'verification smoothly. 3. After receipt of verification forms(hard copies) from IOCL, O/o The Nodal Officer will segregate the forms delivery PO wise. A list in duplicate will be prepared for Customer name and number/Sl. No. of Forms. The forms along with one copy of list will be sent to the PM/SPM of the concerned delivery PO & one copy will be retained in the Divisional Office. 4. The PM/SPM of the designated delivery Post Offices will further sort out the verification forms postman beat wise and hand over the verification forms along with a list (to be prepared again in duplicate as mentioned in para 3 above) to the concerned Postman under receipt. One PA and a supervisor will be nominated for this job. 5. The concerned beat Postman during the course of visit in his beat will make necessary enquiry/survey and fill the form either himself or get it filled in by the customer. He should also obtain signature of the customer/informer as the case may be. In the event, customer/informer does not sign the form, suitable remark " refused to sign" should be noted on the form by the Postman. He should also write his name and sign at the appropriate place in the form. 6. These duly completed forms will be returned by the Postman to the concerned PA/Supervisor who will both cross check the forms. A daily record of forms given to postman and received back will be kept by the concerned PA/Supervisor. The completed and checked forms will be sent back to the O/o The Nodal Officer (along with invoice to be made in duplicate) on weekly basis. one copy of the invoice will be sent along with completed forms to the O/o The Nodal Officer and one copy of the invoice will be retained in the delivery PO as office copy for record. 7. The Divisional Office of East Dn. will monitor the movement of forms to and from delivery offices and maintain necessary records. 8. The O/O the Nodal Officer will raise the bill to IOCL on weekly basis in respect of verified/completed forms. 9. The charges for each Address/customer Verification will be Rs. 55/- + applicable Service taxe, Edn Cess etc. 10. IOCL will pay 80% of the total estimated amount of the no. of customers' to be surveyed/verified through Cheque in favour of Sr. Postmaster, Krishna Nagar HO,

Delhi - 110051. The rest 20% would have to be paid by IOCL on completion of the verification within 7 days on receipt of bill from the Nodal Officer. 11. The Nodal Officer will issue sanction for payment of incentive to the staff as per below mentioned incentive structure: Rs. 5/- for carrying out physical verification. Re. 2/- for checking correctness of each form Re. 1/- for cross checking Re. 2/- for consolidating the work and handing over the forms to IOCL 12. Accounting Procedure & Apportionment of the sale proceeds will be as under:: Out of the Revenue realized @ Rs. 55/- per customer verification, an amount of Rs. 45/- (excluding the incentive charges through this scheme may be booked under the Head "1201008000101" (Retail Post), and @ Rs. 10/- under UCR. The applicable Service tax & Edn Cess will be deposited with the Service tax department. The amount to be paid as staff incentive @ Rs. 10/- will be charged under UCP per contra debit to UCR.
