Event Name: MAD ABOUT MUTTS Event Date: July 14, 2012 Event Time: 11:00am-3:00pm Event location: Animal Resource Center - 7949 S. I-35 Service Road OKC, Okla. 73149
Rescue set up details: Doors will be open at 9:30 am the day of the event. Set up must be completed by 10:30am. Please enter the building through the front doors (east side of the building). Groups are responsible for their own tables, 2 folding chairs will be provided for each 10x10 space. Please hang banners from your table. Event participant parking will be in the fenced lot on the north side of the building. Waiver: I assume any and all risks associated with participation in this event, including, but not limited to falls, contact with other animals and their owners, the effects of the weather, including heat and humidity, traffic, and the conditions of the grounds and any other potential injury. Knowing these facts, and in consideration of The Animal Resource Center's acceptance of my registration, I for myself, my family members, my animals, my heirs, executors and administrators, waive, release, and covenant not to sue and forever discharge The Animal Resource Center, any and all parties associated with this event from any and all claims for damage, or liabilities of any kind arising out of participation in the event even though such liability may arise out of my own negligence or action of that of my animals. This release extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown. Photo Use Waiver: The undersigned further also grants full permission to The Animal Resource Center and/or agents authorized by them to use any photographs, videotapes, motion pictures, recordings, or any other record for publicity purposes. Disqualification of an Animal: For the safety of all pets and people in attendance, please limit the number (dogs) you bring to a 3:1 dog/handler ratio. All adoptable pets must be spayed or neutered, clean, current on age appropriate vaccinations, free of external parasites and not displaying signs of questionable health. We prefer pets to be crated or on leash at all times, no flexi leashes please, use of x-pens is at your own risk. Agents of The Animal Resource Center reserve the right to excuse any pet deemed to be unsuitable for the environment. Approximately 10, 10x10 spaces are available. Secure as many spaces as you'd like for $10 per space. Space is on a first come, first served basis. Please mail form and payment to The Animal Resource Center 7949 S.
