I will attend the Graduation Celebration on June 3, 2009. This is a Drug /Alcohol-free lock-
in. All attendees MUST ride buses to and from event. I agree to abide by all rules and
policies set forth by Main Event and PG Booster Club 2009 and hereby waive and release
them from any liability for injury. I give BC 2009 permission to act for me according to
their best judgment in any emergency. ALL STUDENTS ATTENDING PROJECT

please give serious consideration to the following:

_______ I am able to volunteer time for fundraising activities. (The more members
we have the less fundraising we have to do!! So, spread the word.) You can also

_______ I am able to donate items (i.e. drinks, paper goods, gift cards, movie passes,
calculators, cameras, DVD players, Clock Radios … this list is unlimited. ) for
fundraising activities and/or Project Graduation.

RELEASE OF LIABILITY

I hereby acknowledge the inherent extreme risk in rock climbing, including climbing on artificial surfaces. I realize
that those risks include, but are not limited to: falls from or contact with walls and equipment, bad decision making,
inattention of belayers or actions of other climbers, misuse or failure of equipment, holds which may become loose or
damaged, and freakish accidents which cannot be foreseen. I acknowledge that the above list is not inclusive of all
possible risks associated with the use of the facilities, and/or the sport of climbing and I agree that said list in no way
limits the extent or reach of this release. I voluntarily assume all such risks for said minor(s) and with full
knowledge and appreciation of the danger and risk involved.

I voluntarily agree to assume all risk of personal injury to said minor(s), whether or not under supervision of Main
Event personnel. I hereby knowingly and intentionally waive and release, and agree to indemnify, hold harmless and
defend Main Event, its successors, assigns, officers, employees, and wall designers and builders, hold manufacturers,
lessors, affiliated organizations and agents from all liability for any such damage, injury, paralysis or death which may
result. This release shall be effective even though said loss, damage or injury results or has resulted from the
negligence, wrongful acts, omissions, breach of warranty or strict tort liability of Main Event or the other parties
released.

I acknowledge that said minor(s) is(are) in good health and has(have) no physical limitations that would affect their
safe use of the facilities. I agree they will pay attention to the state of any ropes, anchors and other equipment they
may use, and to advise staff members if they do any damage or notice any damage. I certify that I have read, or agree
to read the posted rules, and agree they will abide by these rules and any future rules, and if staff makes a specific
request of or instruction to them, they will comply.

This release shall be effective and binding upon me and my assigns, heirs, representatives, executors and
administrators. In consideration for the minor(s) listed, for whom I am legally responsible, being permitted by Main
Event Entertainment (“Main Event”) to climb at its facility, portable wall and/or participate in any program offered by
Main Event, including its climbing school, I agree to the following waiver and release and make the following
representations on their behalf:
I understand that this release is a legally binding contract. No oral representations, statements or inducements
apart from the above written agreement have been made. I expressly state that I have read, understand and am
familiar with all its provisions and that I sign it of my own free will. I further expressly agree that this release,
waiver and indemnification agreement is intended to be as broad and inclusive as is permissible by the laws of the
State of Texas and that if any portion of this agreement is held to be invalid, it is agreed that the balance shall,
notwithstanding, continue in full force and effect. Should Main Event or anyone acting on their behalf be required
to incur attorney fees or costs to enforce this agreement, I agree to indemnify and hold them harmless from such
fees and costs.
