Failure Modes and Effects Analysis (FMEA)

FMEA is a systematic, proactive method For Evaluating A Process To Identify Where And How It Might Fail And To Assess The Relative Impact Of Different failures FMEA includes review of the following: 1. Steps in the process Failure modes ( What could go wrong?) 2. Failure causes ( Why would the failure happen?) 3. Failure effects ( What would be the consequences of each failure?)

Problems study based on quality concepts

Medical record department complaining from increasing medical file deficiencies .

Step 1 : Identify (find) the problem Control chart P OR U chart

Sample size represent : TIME ERRORS

To know the stability of the process & the average

If the team decide to use P chart they collect the data as follow:
TOTAL

If the team decide to use U chart they collect the data as follow:
Sample size represent :

ERRORS

SAMPLE

Results : there is medical file deficiencies ratio 30% or 10 deficient file / w as average

Step 1 : Identify (find) the problem Step 2 : Formulate new problem statement

Total Medical file deficiencies represents 30% or 10

file / w and the hospital target is 0 defect Definition: Medical file deficiency uncompleted any field no date or time no signature missed any form

Step 1 : Identify (find) the problem Step 2 : Formulate new problem statement Step 3 : Organize improvement team
Using flow chart (ideal)

IF it successes
Published the result of the pilot study Implement for all departments Established indicator Use control chart to monitor the process

If no successes
