Combining of Heart Failure
/Cardiac Rehab Services

In June 2006 Heart Failure and Cardiac


Rehabilitation Services at Shrewsbury
&
Telford Hospital NHS Trust (PRH &
RSH)
combined four small teams into one.
Identifying the Need for
Change
 Four separate teams across Shropshire
 Not all skills in the department being
utilized
 Referral of patients to both Heart Failure
and Cardiac Rehab Teams
 Duplication of roles
 Differences in cross site working
 Each individual team was small in staff
numbers, combining them resulted in one
large more efficient/effective team .
