These areas for improvement are applicable for planned


jobs in normal circumstances. In the case of Emergency, stakeholders are to use their best

1. Permits Authorization

Applicants will submit all planned work permits at least one week

ahead (D-7 days). To place them in dedicated slots from Monday to


Friday, depending on which day they intend to start work.

will level-out- the work load of long-lead time jobs (ie.

Job to be executed longer than d + 7 days) E&SB work permits take priority with dedicated slot. No other changes to process; ie. AGSI to sign off work permits daily and ensure JHA is also sign/check by Operation

2. Operation Preparation (D 1)
Applicant to submit permits for next days job to Plant Managers before 1600hrs daily. No change to process: Plant Managers will continue providing Shift Instructions to shift team to prepare for next day work execution (e.g. switching pumps & isolation) After approval by Plant Manager, permit will be put in Next days permit slot for night shift team to prepare. Night Shift ASCs to pick up permits in next day permit slot and prepare accordingly

Operation Preparation (D 1)

If unable to prepare equipment for various reasons ((e.g. passing

valves, single pump), ASC to capture reason in ASC log and place
red sticker on permits, and put it back in Next Day Permit slot for Applicant to collect. Applicant will return the next morning to pick up his issued/rejected permit

3. Permit Issuance stage and job execution

Permits issuance to start work: begin at 0830 hrs

(after operation SH, SoSO and STM)

Possibility of limiting issuance time from 0830 0900hrs, to enable gas testing to proceed without interruptions

Possibility of not issuing jobs that was meant to start in the


afternoon

In house contractor normal working hours shifted to 0830hrs

1730hrs
For LOTO isolation, PTW issuer to radio electrical person (Channel 8)

to Applicants?
