Classification

Employee was observed by JGC HSSE personnel walking on the pipe Serious Safety Violation where he could exposed to a fall of 2m. Employee was spotted used improper way to gain access to vertical ladder where he could exposed to a fall of 4m. Employee A strangulated and pushed employee B during an argument while boarding a lorry. Employee B sustained 2 scratch marks on his neck and and then retaliated by pushing employee A. (Employee A had past offence of hitting another co-worker with an argon hose during a work related discussion and was suspended for 2 working days in March 2013). 3 fitters and a foreman were preparing for bolt tensioning activities. Foreman checked work location and turned on air supply of air compressor (AC-053) which had pre-installed hoses with R-pin and whip arrestor. Serious Safety Violation Serious Safety Violation

Suddenly the attached air compressor hose dislodged together with the 2 hose clamps. The hose was still retained by the whip arrestor and the coupling still intact due to the presence of the Rpin. The hose clamps landed 2 metres away while the other landed at about 6metres away.

Description
Employee was spotted using hand-phone at a non-designated area (PEC canteen). Employees were observed by JGC HSSE carrying out welding task standing outside of scaffold platform. Employees were exposed to fall more than 2 metres. ( Both Employees were tied-off 100%). Supervisor filled up the Pre Task Talk (PTT) form and conducted PTT. He was not able to sign the PTT form as he had acknowledged another PTT form and he asked another supervisor who was not present at the PTT to sign off the PTT form. 2 fitters were tasked to carry out air-blowing activities and carried out preparatory works. Prior to activating air-compressor (Weldman - 2378), fitter (A) checked work location and air compressors hose & fittings. In addition, he also informed his experienced partner (fitter B) to carry out a final check. Fitter B concluded that air compressor was in order and informed fitter A to activate machine. Upon releasing the lever, air-compressor hose dislodged from the equipment and landed on a scaffold barricade 1 metre away. The hose coupling was still intact with the equipment. (It was later discovered that hose clamp was not tightened and the whip arrestor anchored at the hose coupling).

Description
Employee was working on a red tag scaffold platform and was observed by PEC HSSE personnel. Employee was involved in bolt tightening activity for permanent staircase handrail. The activity required use of a crow bar for alignment purpose. He was using a crow bar (400mm) attached with lanyard for the task. While carrying out the task, he failed to tie-off the crowbar causing it to drop to ground (est.3.5metres). Employee was observed not wearing face shield while grinding unistrut channel against a high-speed cutter's cutting wheel. His hand was also observed about 1 inch away from the rotating wheel. Employee was observed having hand-phone at a non-designated area (PEC Store Area- Laydown 2) Employee was spotted having handphone while at nondesignated area (PEI Fabrication yard- Laydown 1).

Description
Employee was caught using 6 inch grinder without complying with additional required PPE (ear muffs). Employee was observed carrying tools in the tool bag without lanyard at height. Tools were not used at that time. (site requirement-all tools to be attached with lanyard) 2 Employees were embroiled in a verbal argument related to work activity. Both of them were about to exchange blows when a PEC HSSE who was nearby managed to separate them. Employee was observed working at the edge of scaffold platform without 100% tie off his safety harness. In addition, he was observed removed his hand gloves and safety glass at that time. Description
Employee was observed by JGC HSSE carrying out welding task standing outside of scaffold platform. Employee was exposed to fall more than 2 metres. (Employee was tied-off 100%). While activating an air compressor, employee did not ensure that whip arrestor and hose clamp were correctly fixed to the air hose causing the air hose to be dislodged upon releasing air from the machine. Supervisor filled up the Pre Task Talk (PTT) form and conducted PTT. He was not able to sign the PTT form as he had acknowledged another PTT form so he asked another supervisor who was not present at the PTT to sign off the PTT form. Supervisor acknowledged and sign on PTT form without being present during the PTT and analyzing task stipulated on the form. Employee conducted a final check on air compressor and hose fittings prior to activating machine but failed to re-adjust whip arrestor and hose clamp connection causing the whip arrestor to be dislodged upon releasing air from the machine.

-Poor housekeeping at A340. Waste materials left all over the place. - Observed poor housekeeping at steam tracing area. - Unused chemical (molykote) left unattended on the permanent structure.

ROOT CAUSE
Job planning did not include housekeeping.

MITIGATION PLAN
To emphasize in tool box meeting and increase inspection frequency on elevated areas.

- Fail to provide adequate spark containment. Sparks were flying out from the fire box. - Fire extinguisher was far from hot work area. - No fire watchman when worker doing the grinding.

Understanding of risk less than adequate.
