EXPENDED AND EXTENDED ROLE OF NURSING

INTRODUCTION
Definition of nursing

Nursing encompasses, autonomous and collaborative care of individu of age families, group and communities, sick or well and in all setting. Nursing includes the promotion of health, prevention of illness and care of ill, disabled and dying people.Advocacy,promotion of a safe enviroment, research, participation in shaping health policy and in patient and health systems management and education also key of nursing role.

Care giver Activities that included client/patient physically and psychologically. Decision Maker. Uses critical thinking skills to make a decision to provide effective care.

Health care Coordinator. Serve as the primary coordinator of all other professional services commited to the care of the patient

Communicator. Central to all nursing roles and activities

Educator Explains to patient concepts and fact about health, demonstrates procedures as self care activities.
Counsellor. Helping patient to recognized and cope with psychological and social problem.

Client protector and advocate


Responsible for defining defending and promoting the right to patient.

Extanded Role Meaning to strech out.Means extension of nurses roles which were previously undertaken by doctors or other healthcare professional. Expanded Role. Meaning spread out.Means an expansion of nurses roles within the boundaries of nursing education, theory and practice.

Policy Implication . Several barrier identified like to diagnosis and treatment. Nursing Shortage. Affects patients care because the number of patients to one nurse is increasing there for decresing quality care.

CONTINUE Gain Experience. Competence as being low when first assume due to the new of the role. Nursing Scope Practice. Radcliff (2000) suggest that nursing has missed an opportunity to strengthen its own core value by taking on expended roles like endoscopies and pre operative assessments . The professional nurse is a caregiver,teacher,client advocate, quality of care evaluater, manager, consultan and case manager.

IMPLICATION OF EXPANDED AND EXTENDED ROLE


1) Collaborative Practice. Positive way due to the support and understanding received from physicians and nurses. 2) Continuing Professional Development Benefit of continuing education is helping nurses feel more secure in the roles.

3) Expending Health Promotion. Full utilization of nursing service is dependent upon gaining public and professional recognition for the role.

4) Implication for Quality Care.


As with urban health care, the limited rural research that does exist indicates variability of care quality. This circumstance underscores the need for nurses and others with interest and expertise in rural health to further expand knowledge in this largely ignored area. One particular area needing nursing inquiry is the extent to which rural health care delivery reflects activity and progress toward achieving the six aims for improvement. (Institute of Medicine,2006)

CONCLUTION.
In Malaysia, the extended and expended roles of nurses into advance and specialized practice differ to some extent from that in developed countries. They differ in relation to the aspects of scope of work and recognition. The scope of work differs because Malaysian nurses prescribing right is limit.The nurses in health care can prescribe drugs that are common like multivitamin ,antiphyretic for maternal and child in health services. The progression of nursing practice and recognition of nursing within the Malaysian contex is slow. This has partly contributed to the inability of nurses to progress to advanced practice and specialization. A nurse needs to have at least a Master degree in an advanced and specialized area.
