Background of The Problem


In the Asean countries, Indonesia has the highest of mother death rate and prenatal. It means that the totally qualifiead improvements in the aspect are still necessary. Being predicted that the childbirth rate in the Indonesia is about 5.000.000 / year, it can be described that the mother death rate is 19.500 200.000 annually or 26 27 cases minute ( Manuaba, 1998 ). Syahri (2002), in his research about maternal care program, states that the high rate of family and social care. The lack of it can lead to the three kinds of late action, including : recognizing the danger, taking to the referred place and giving treatment in the referred place.

The men participation in the reproduction health is so low. In KB program only 2% participation of men. Mother date rate (AKI) in indonesia is high; 373/100.000 succesful childbirth, in which 17.000 fatal cases occur because of three kinds of late treatmen and 68% of childbirth is without accompaniment of husband. The level of high fatality childbirth and low quality of woman reproduction come from the patriarchal custom (Anonymous, 2004). Childbirth process is one of factor causing an anxiety because it is not only somatic but also psychosomatic (Kartono, 1992).

Based on the previous survey conducted in November 28, 2005 in Tegalrejo public health center of husband accompaniment. However, there are still cases in which the anxiety level arises because many childbirth processes are without the accompaniment of husband or family. Tegalrejo puskesmas is one public health center under apervised by Dinas Kesehatan kota Yogyakarta with significant visit level of pregnant mother, it is about 1093 in 2005 or on the average 91 pregnant mother per month with one dead baby in 2005 (Sistem Pencatatan dan Pelaporan Terpadu Puskesmas (SP2TP) Puskesmas Tegalrejo 2005).

DISCUSSION
The result shows that the whole respondents get the high scores (14-25). in husband accompaniment, it is obtained that the high anxiety in respondents accompanied by their husband is lower 16,7% than those without. In the anxiety level of the respondent without husband accompaniment is 40%. The anxiety on pregnant mother before childbirth process comes from the worriedness of unwanted situation both by anxiety, unconcentration, trembling, easy to get angry, cold sweat, stressed, frequent urinating, upchucking are kinds of process happens. The mother feels that it is hard to conduct the childbirth and consequently, some of them say that they are ubwilling to have babies anymore. By using the statistic test Chi-Square, it is obtained that husband support system during the childbirth process stage I correlates significantly with the mother anxiety level (r=15,765; P=0,000 < 0,05). It assumes that there is a meaningful relationship between husband support system and the mother anxiety level in childbirth process stage I. NEXT

Barrier Factor
1. Not all normally childbirth mother accompanied by their husbands can be used as respondents because their processes happen at the sametime, so yhe writer cannot observe all the processes. 2. Not all husbands can be regarded as respondents because some of them are just waiting or accompanying outside the childbirth room. It may be caused by the lack of knowledge about the importance of husband support system

CONCLUSIONS & SUGGESTIONS


1. The husband accompaniment in childbirth process shows that all respindents get the high scores (14-25). It exposes that all respondents need their husband beside them during the process. There are different mother anxiety level is between mother not accompanied by their husbands and those with their husbands. The absence of husband support System enhances the level of mother anxiety. There are some factor causing the anxiety during the childbirth process;they are:painful contraction, crying, anxiety, unconcentration, trembling, easy to get angry, cold sweat, stressed, frequent urinating upchucking, reluctant to eat, and frequent calling for the paramedic, and hard to follow the instructions when the childbirth is processing.

SUGGESTIONS
1. For the research.
It needs condusting a wider-scoped research by considering other variables influencing the anxiaty.

2. For the Paramedic.


a. It is hoped that they should apply therapeutic communication because it has very important role in every maternal service processes, so it can keep the clients satisfied and reduce their anxiety. b. It is necessary to give guidances to mother about how the childbirth processes happen and what preparation should be done in order to minimize the level of anxiety.
