Identify hazards within your own working areas!! Work on Electrical equipment: 1 of 3 Isolation of fan Identify Potential Hazards! 1/3 Work on Electrical equipment: 2 of 3 Activity Isolation of fan Preparations Work Permit, Personnel Protective Equipment (PPE) Personnel Two experience electricians Circumstances Faulty cubical breaker Similar problem had been solved earlier by changing the handle. Action The electricians received information on how to change the handle. The handle was replaced but still did not work 2/3 Work on Electrical equipment: 3 of 3 Actual / potential consequence 1 and 2 degree burn (on hand)/(DAFWC) serious injury What happened One of the technicians opened the door and used the old handle directly to the breaker operation axel and turned the locking arm The locking arm came too close to the live part of the fuse and created an electrical short circuit Direct causes 1-10 Short circuit 3-1 Lack of knowledge of hazards present System causes 13-1 Inadequate assessment of needs and risks (tools & equipment) 15-10 Inadequate communication of safety and health data, regulations or guidelines 3/3 GRACE Group Risk Assessment Conversation & Experience transfer Identify hazards within your own working areas!! Work on Electrical System: 1 of 3 Working on an 4160 V overhead Identify Potential Hazards! 1/3 Work on Electrical System: 2 of 3 Activity Working on an 4160 V overhead Preparations Work Permit Personnel Two electrical crews Circumstances Failed meggar test were found on 2 of the 3 phases Repairs were made to the 2 phases Following repairs, one phase still failed the meggar testing Action It was determined that the linemen would be required to assist in investigating the failure Since it was break time and it appeared as if it might rain, the transformer cover was put back with two screws. 2/3 Work on Electrical System: 3 of 3 Actual / potential consequence Electrocute / Fire What happened The wireman crew foreman thought that the work was completed, so he told them to install the cutouts and energize the transformer. Immediately after being energized, one of the splice connections failed and the 4160 volt feeder tripped out on ground fault. Direct causes 3-1 Lack of knowledge of hazards present 7-3 Work expose to energized electrical system System causes 13-1 Inadequate assessment of needs and risks (tools & equipment) 14-4 Inadequate enforcement of Policies, Standards, Procedures 15-10 Inadequate communication of safety and health data, regulations or guidelines 3/3
