BOC Official Receipt (processing fee)
List of Expected Imports, including, estimated volume and values for the incoming 12 months
Corporate Secretary Certificate or Special Power of Attorney for Designated Signatories
Two (2) valid government-issued I.D. with of the applicant and responsible officers
Customs Accreditation Program: For Importers Documentary Requirements 3 NBI clearance of applicant (in case of corporation) or the sole proprietor
Printed CPRS profile of the importer and updated email notification of STORED CPRS profile
Company profile (for corporation) and trade profile (for sole proprietors)
Personal Profile of application and officers (for corporation)
Previous Certificate of Accreditation, if any.
