Discharge Planning

Medication
• Encourage the patient to have a strict compliance with regards to the
medication to attain therapeutic effects.
• Give adequate instructions to the significant others about the
importance of the following medications and dietary regimens so that
the patient’s condition can remain stable as soon as possible.

Exercise
• Encourage postpartum exercises to fasten the involution and systemic
healing.
• Encourage walking to endure musculo- skeletal functioning.
Stretching
• To stretch the muscles, the arms and legs should be moved in ways that
produce slow, steady pull on muscles to keep them loose.
Strengthening
• Work specific muscle groups to enable for the patient to support the
body better and increase function.
Positioning
• Abduction of pillows, sitting recommendations, a little bed rest and
frequent positioning of the patient as much as possible.
Treatment
• Antibiotics given orally every 12 hours which is the Co-Amoxiclav 625g and
every 8 hours which is the Metronidazole 500g.
• Analgesics given orally which is the Ketorolac 10 mg every 8 hours. This is for
the pain due to the incision at her abdomen during cesarean procedure.
• Relaxation Therapy
• Counseling Therapy to address the emotional needs of the patient due to the
death of newly born child.

Health teachings
• Practice good hygiene.
• Practice good preventive measures by eating proper diet.
• Getting regular exercise and plenty of sleep.
• Seek clinical advice if abnormal bleeding occurs.
• Use of supporter or splint around the abdominal incision is encouraged, to
prevent unsealing of suture when patients coughs or does demanding works.
OPD Follow- up
• Instruct the patient and family to return ti their attending physician for
scheduled follow up visit.
• Instruct the patient to continue taking the medication as prescribed and
return to Northern Mindanao medical Center for scheduled follow up by
Dr.Gotil.
• Advise patient and the family to report to the physician if any recurrence or
severity of symptoms, any adverse effects to the medication, and any
development of complications.

Diet
• Advice to continue low salt and low fat diet like porridge chicken soup.
• Encourage intake of foods rich in protein and high in calories for fast healing
of the wounds. (e.g. Milk , rice , meat , fish)
Spirituality
• Encourage the patient to read the Bible and pray to God always, ask for
guidance and pray for the healing and restoration of health.
• Ask the patient to reflect on the Bible Scripture, “For I will restore
health to you and heal you on your wounds.” says the Lord. (Jeremiah
30:17).
• Always seek spiritual advice and go on Sunday masses.
