Safety designation of different safety members, and their area of duty.


There are two designated hospitals, San Jose Hospital & Calamba
Medical Center. But the hospital is in case to case basis. It means that if
the injury doesnt need that much medical attention, the patient is
brought to Calamba Medical Center because it is a public hospital.
The company nurse should accompany the patient and will check if he
can go back to work. If it is said that the patient should rest for certain
number of days, and he wants to go back to his work earlier that said
days, the nurse will decide.
The committee will evaluate if workers with disability or certain illness
are still efficient with their work.
There will be a budget for the Safety Department in 2016 which will
include the Physicians professional fee, free medical exam for new
workers, annual medical exam, random drug testing, and other safety
programs.

It is also said that safety should be implemented at night


shifts because workers are seen not wearing proper uniforms.
We should start creating policies and penalties for safety.
Orientations should be conducted regularly to workers to
address these policies.
In cases of not work-related accidents, the companys
responsibility includes the inside of the gate of Filinvest
Techno Park and staff houses.
There should be First Aid Trainings for First Aiders.
Check for the chemical exposure from the company nearby,
and address to them about it.
One reason for unsafe practices of workers is the delay of
salary.

It has been agreed that every worker should join the weekly
exercises / toolbox meeting.
In our companys policy, Section 4.17 Smoking in prohibited area/s
didnt tell that our company is a NO SMOKING ZONE, but according
to GAJ, LMC would like to make this place a NO SMOKING ZONE.
Therefore, it has been agreed to revise first the policy about this, and
while waiting for its revision, smoking area (s) should be allotted.
Currently, there is one already installed, it is located near the Gate 1,
while the other will be planned where should be properly located.
With regards to cleaning of bottles and tansans, the one who sell
should be the one to clean about this.
The garbage bags located in different areas of fabrication should not
have biodegradable wastes. These wastes should be disposed
immediately in the segregation garbage bins located at both ends of
the fabrication bldg.

Proposal about gases should be monitored to Mr. Jeffrey Peralta


Proposal about PPE Monitoring should be monitored to Mr. Efren Mendoza
A check list of people that will be issued a spectacle will be done by Mr.
Wilson Balucan.
A proposal about the locker room should be made by PJAA. As of now,
bags can be brought to the work area, while the changing of clothes
should be done in the CR.
A proposal about hazardous storage and bin should also be made by PJAA.
And the proposal should be monitored to Mr. Allan Vitor.
Cooking is an issue inside the fabrication. It is approximated to have more
or less 20 rice cookers that are used for cooking. With that, Mr. Wilson
Balucan will look for an area for this, to relocate all the cooking activities,
to avoid short circuit and to have good housekeeping in our fabrication
area.
With regards to the accident, a follow up report should be followed.

For the general discipline, the policies are translated into Filipino language. Only
sections 4.17, 4.21, 4.22, 4.23 and 4.24 can now be explained to the workers, and
will be implemented immediately, since it has been explained already. Safety
Committee members of different areas are in charge of their areas.

When the member saw the violation, he will immediately approach the worker, and
will remind about his / her violation. The member will make a report, signed by him,
and he will submit it to the Safety Engineer. The SE will pass it to the department
head of that worker and have it signed by him/her before submitting to the HR. The
HR will apply the corresponding disciplinary action for that offense.
