Electronic Health

Record System

Electronic Health Records

(EHRs)
Refers to the systematized
collection of patient and
population electronically-stored
health information in a digital
format. These records can be
shared across different health
care settings. Records are shared
through network-connected,
enterprise-wide information
systems or other information
networks and exchanges.

The EHR includes all relevant

information pertaining to the health
of an individual including his/her
demographics, problems, prescribed
medications and treatment plan, vital
signs, past medical history,
immunizations, laboratory and X-ray
reports or any such information that
is required to be easily accessible by
any healthcare provider at any point
of time in the lifespan of the
individual.

A summary of the patient's

medical history, and
documentation of each event,
including symptoms, diagnosis,
treatment and outcome.
Relevant documents and
correspondence are also
included.

Features of an EHRs are as

follows

Wide scale computer-based patient

record
Complete collection of individuals
health related data
Database format
Data for HER stored in clinical data
repository
All data from all different systems
Data can be integrated to obtain a
different level of information
Each element or data has to be defined
Closed and secured system

Today, EHR system are

increasingly recognized to meet
clinical and business needs in
healthcare by capturing, storing
and displaying clinical
information when and where it is
needed to improve patient care,
and maximize cross-patient data
analysis.

Benefits of EHRs
They enhance patients safety
Real time information regarding
patient care
Eligibility of documentation or
orders
Access to medical information
remotely
Better security of medical records
Archives of medical records

The Philippines government is

gradually mobilizing its rural health
units (RHU) to adopt an electronic
patient record system in their offices.
The UP Manila National Telehealth
Center has initiated and developed a
computerized patient record system
known as Community Health
Information Tracking System (CHITS)
aimed to provide Filipinos quality
health care and management even in
hard-to-reach areas.

Community Health
Information Tracking
System (CHITS)
Community Health Information
Tracking System (CHITS) is a free
open source software electronic
health record system for local
government health centers in
the Philippines.
CHITS was created by Dr.
Herman Tolentino in 2004
through a grant from the
international Development

How does CHITS work in a health

center?
As the patient comes in, his/her record is
retrieved in seconds. If the patient is new
he/she would be registered that would take few
minutes.
CHITS is accessible in all departments within
the health center so once the patient
information is processed in the admission area,
the doctor and the other health provider are
promptly informed of the patients chief
complaint ,vital sign and history without
having to wait on the patient.

Open Medical Records System (Open

MRS)

Another electronic health record

program introduced by The
National Telehealth Center is the
Open MRS.
It is a software platform and a
reference application which
enables design of a customized
medical records system with no
programming experience.

It involves minimal use of free text and

maximum use of codes and standard
information.
At its core is a concept dictionary which
stores all diagnoses, test, procedures,
drugs and other general questions. It is a
client- server application and works best
in a networked environment.
Regenstrief Institute and Partners in
Health created OPEN MRS in 2004 as an
open source medical record system
platform for developing countries.

Open MRS was created in 2004

as an open source medical record
system platform for developing
countries by Regenstrief Institute
and Partners in Health.

What is Medical Record?

It contains the patient's personal
details (such as name, address,

information of an individual that
is created, gathered, managed, and
consulted by licensed clinicians
and staff from a single
organization / practice who are
involved in the individuals health
and care.

Electronic Medical Record

It is a computerized medical
record
It is used to deliver health
care information
It is used in hospitals,
nursing homes, insurance
companies, and other
healthcare facilities.

Challenges of an EMR
It is expensive and takes time to
implement
It requires retraining of staff
Patients medical records could be
compromised
System could crashed
