KEY MANAGEMENT STANDARDS

1. LEADERSHIP AND ADMINISTRATION


Effective leadership and program administration are to the success of Loss Control Management
Program. Management leadership lays the foundations upon which the program is developed.
Management must.

- Issue to all employees a Health, Safety and Environment


Policy Statement which includes the
arrangements for the Implementation of the Safety Management System.
- Provide Management Standards and Procedures specifically addressing loss prevention or Loss
Control Management for all managers.
- Put in place an Annual Loss Control Management Program designed to meet Companys HSE
goals and objectives.
- Designate a Loss Control Management Program Coordinator to oversee the Implementation of the
Loss Control Management Program .
- Develop an audit program specifically to measure the level of compliance with the Loss Control
Management Program and evaluate effectiveness. (CKPI)
- Actively participate in the development and implementation of the Loss Control Management
Program and have their respective responsibilities defined in their job descriptions. (SQM)

2. MANAGEMENT TRAINING (LEADERSHIP TRAINING)


Management training shall provide the knowledge, motivation and skills necessary to
manage the Loss Control Management Program.
-All managers shall be trained in the Safety Management and Loss Control Management
techniques which emphasize the need for prevention and control of loss.
-All superintendent and supervisors shall be similarly trained in the implementation of
Safety Management and Loss Control Management Techniques.
-All new management members will be given an induction program which will include a
briefing on the Loss Control Management Program, including the Safety Management
System.
3. PLANNED INSPECTION AND AUDITS
Planned inspection and audits involve the systematic examination of
Company
Organization and activities which will identify hazards and Loss Potential items, and will
also provide a means of feedback to management of the effectiveness of the Loss
Control Program, it Policies, standards and procedures.
A general and management inspection and audit program will be developed in
accordance with the planned inspections and auditing policy procedures.
- The responsible managers shall ensure that all inspections and audits are properly
reported and that a follow-up procedure, to ensure that corrective actions are taken, in
place and implemented.

4. TASK ANALYSIS AND PROCEDURES


Task analysis and procedures involve the systematic review of the tasks required to be
performed and their breakdown into components to establish safe and effective work
practices and procedures for each component.
- Task Analysis may be conducted at the discretion of area managers. (See also Job/
Task Observation.) An inventory of all tasks carried out in operation would be developed
and evaluated to determine which are critical tasks. These critical tasks would be
reviewed and procedures developed for them.
5. ACCIDENT/ INCIDENT INVESTIGATION
Accident/ Incident Investigation involves the methodical examination of the event to
identify the Immediate and Basic Causes such that corrective action can be taken to
prevent recurrence.
- All accidents, incidents and environment damage shall be reported and investigated
according to the Accident/ Incident Reporting and Investigation Procedures.
6. JOB/ TASK OBSERVATION
Job/ task observation involves the systematic observation of employees in specific tasks in order to
determine that those tasks are being carried out in accordance with the established procedures.
- Job/ Task observation may be conducted at the discretion of the Support and Operations
Managers.

7. EMERGENCY PREPAREDNESS
Immediate effective emergency response reduces the actual and consequential losses in
advance of any emergency so that effective response to any emergency situation can be
made.
- An Emergency Control Manual containing the Company emergency procedures and
plans shall be maintained up-to-date and available for all operations.
- An Emergency Preparedness Coordinator will be appointed to be responsible for the
administration and exercising of the emergency procedures and plans reporting directly
to the Vice President/ General Manager or Vice President HSE Integrity.
8. ORGANIZATION RULES
The establishment of general guidelines in Safety and Health such the probability of
incurring Loss
as the result of an Unsafe or environmentally damaging act
(Substandard Acts & Conditions) being committed by an employee is considerably
reduced .
- Management shall ensure that such regulations, procedures, rules and practices, which
are considered necessary for the safe conduct of all operations, are in places, up-to-date
and fully understood by all who use them.
- All employees will be given access to and properly informed of Companys Safety
Manual (if not personally given a copy). (HSE M.S. REF. MANUAL) (HSE M.S./

9. ACCIDENT/ INCIDENT ANALYSIS


The systematic analysis of accidents and incidents to identify relevant information for
management and staff to use in their efforts to control and prevent Accidental Loss.
- The HSE DEPARTMENT will analyze all accidents and incidents to identify trends and
will report to the Vice President/ General Manager or Vice President HSE Integrity
Quarterly.
10. EMPLOYEE TRAINING
It is necessary to formally provided employees with the competency and proficiency to
perform their work in compliance with the standards established by Company.
- An ongoing Safety and Vocational Training Program for each category of occupations
will be developed and maintained by the HRD-Training Group, based on
recommendation from the Vice President/ General Manager or Vice President HSE
Integrity.
- The Training program will be reviewed and evaluated annually by the Vice President/
General Manager or Vice President HSE Integrity.

11. PERSONAL PROTECTIVE EQUIPMENT (PPE)


Where it is not possible to remove all sources of hazards to employees it is necessary to
ensure that suitable personal protective equipment is provided to protect employees
from either occupational or physical injury.
- PPE will be provided to all employees where ever there is actual or potential exposure
to hazards which cannot be removed.
12. HEALTH CONTROL
An occupational health program ensures that all potential health hazards are recognized
and controlled.
- An occupational health program ensure that all potential health hazards and to provide
conformation that they are being satisfactorily addressed will be conducted annually.
13. PROGRAM EVALUATION SYSTEM
A program evaluation system is a means to accurately measure performance in the Loss
Control Management Program and to evaluate the results.

Every year the HSE DEPARTMENT Head will make arrangement to complete a
comprehensive audit of compliance with the standards as set by the Loss Control
Management Program.
- A Loss Control Plan will be developed by the Loss Control Coordinator or HSE
Department Head to meet program needs , as indicated by the audit, and this will be
submitted to the Executive Committee for approval.

14. ENGINEERING CONTROL (MCC)


Engineering Controls for Health, Safety and Environment are able to effectively minimize
Accidental Loss Potential by ensuring that appropriate designs and specifications are
used for material and equipment. (BENEFIT = REVENUE EXPENSES - BUSINESS LOSS) *
- Engineering procedures and Management directives will be issued by the Projects and
Construction Manager ensuring formal Health, Safety and Environmental control
reviews at the concept and design stage of all new developments or modification
projects.
15. PERSONAL COMMUNICATIONS
Personal communication between supervisors and staff enhance Safety Performance.
- All superintendents and supervisors will receive formal training in communications skills
necessary for successful one-on-one communication.
- All new or transferred employees will receive initial job induction and training from
supervisors.
16. GROUP MEETINGS
Group meeting are an important method of encouraging successful manager-supervisorstaff communications. This is a two-way dialogue which encourages employee
participation and therefore the development of team spirit.
- Structured group meetings will be held in all locations at least monthly and the meeting
contents recorded.

17. GENERAL PROMOTIONS


General promotions stimulate Health, Safety and Environmental awareness and influence
employee attitude by the use of promotional and incentive techniques.
- The HSE DEPARTMENT Head will ensure that each publications of in-house
communication journals will contain materials relative to occupational Health, Safety
and Environmental issues.
- An ongoing house keeping program will be conducted throughout the operation which
will stress material conservation as well as cleanliness. It will be supported by regular
inspections.
- Accident statistics will be distributed by the HSE DEPARMENT every three months.
These statistic will go to all levels of Management according to an agreed circulation list
18. HIRING AND PLACEMENT
All persons hired or placed in a job must meet minimum physical and competence
standards determined specifically for the job.
- All new employees are required to complete a medical exam prior to employment at
which any specific medical requirement of the job will be recognized.

19. PURCHASING CONTROLS (CSMS) (ISRS 19.2)


Workplace losses resulting from substandard equipment *, materials and services can be
controlled through procurement programs and procedures.
- Formal procurement procedures, which include HSE considerations will control the
acquisition of all materials, services and supplies .
- ISRS 19.2 for CONTRACTORS Selection must be Implemented to get HSE
Management System Performance for more effectiveness to minimize LOSSES.

20. OFF-THE-JOB SAFETY AND HEALTH


The encouragement of employees to take their safety and health training and information
into their domestic and recreational environments.

21. ENVIRONMENTAL CONTROLS


Environmental controls are generally reflected in the preceding elements. However, the
following specific items will be in place.
- An environmental screening and evaluation will be performed in advance of any new
project or operation .
- An extensive baseline assessment will be performed in new areas, followed by regular
routine monitoring and follow up.
- Procedure to control and minimize wastes and to control site clearance and restoration
will be implemented and documented.
22. CONTRACTORS MANAGEMENT (ISRS 19.2) *
A system will be established to apply relevant aspects on the above health, safety and
environmental management system elements to contractor control for all of
CONTRACTORS (ISRS- Element 19.2).
