RADIOLOGY IN AIDS

DR MOHAMMAD ABURUMMAN
RADIOLOGY RESIDENT
MOH Hospitals

Introduction
Most of the manifestations that have
benefited most from imaging modalities are
those involving the central nervous system,
thorax and abdomen.

CNS Manifestations
-Direct consequence of the HIV virus.
-AIDS dementia complex
-immune reconstitution inflammatory
syndrome
-Opportunistic infections and neoplasms in
the immunocompromised host.
-Drugs related complications .

Opportunistic related :Toxoplasmosis is most common

Post-contrast CT scan. A very large, single, ring-enhanced parietooccipital lesion with a large mass effect and peripheral oedema.

TOXOPLASMOSIS

contrast-enhanced scan: the three lesions seen show

typical ring-like enhancement of deep lesions with surrounding edema

Lymphoma
Large homogeneously enhanced periventricular mass with mild
peripheral oedema and mass effect .

PRIMARY CENTRAL NERVOUS SYSTEM LYMPHOMA

Before therapy
brightly enhancing,
multifocal,
periventricular lesions

After therapy

HIV Encephalitis
periventricular hypodensity
no mass effect

Pulmonary manifestations
PCP

Diffuse infiltrates

Bilateral thin walled cysts and

pneumothorax

.Adult male HIV patient presenting with cough

.Pneumocystis carinii pneumonia

,Typical bilateral ground-glass shadowing

cystic change in the right upper lobe
.and a left pneumothorax

Clinical features:43-year-old HIV positive woman with cough

Multiple, bilateral nodules

( arrowheads)
are present and are
associated with
a peripheral wedge-shaped
.region of consolidation )arrow(

Invasive aspergillosis

yr old man with advanced aids.H/o severe cough and purulent sputum 48

Bilateral lower zone

Patchy consolidation

Tuberculosis in advanced AIDS

CASE 14:

4 year HIV Positive child with cough

multiple, ill-defined &

Occasionally
confluent nodules
throughout the lungs.

Varicella-Zoster virus pneumonia.

:CASE 15
HIV Postive male with severe respiratory symptoms on ventilator

, hilar and mediastinal lymphadenopathy

with bilateral
.widespread air space consolidation

Cytomegalovirus(CMV) pneumonitis complicated by

.Adult Respiratory Distress Syndrome
. CMV was isolated from nasopharyngeal aspirate

Mycobacterium TB shows consolidation in the

upper part of the LUL containing several cavities

KS

KAPOSIS SARCOMA

K.S ,HRCT scan

cavitations

Abdominal manifestations
manifestations of AIDS are numerous and
include parenchymal, lymph nodes and
primary gastrointestinal and urinary tract
disorders.
Malignancy is common and KS is the
commonest malignancy followed by
Lymphoma .

AIDS-related KS of the liver is the most

common intrahepatic neoplasm in patients
with AIDS and was found in 34% of cases
in the autopsy.

Candida oesophagitis

AIDS related Lymphoma

Ercp -AIDS related cholangitis
