This agency project will be intended for my supervisor. I will be implementing a solution to
an ongoing issue my supervisor has been having with the organization and location of the
available community resources for the residents of the facility and their family members. My
plan is to create a documented file that will better help my supervisor with educating the
residents and their families on the many different resources that are made available to those
in need; whether they are within the facility or at home. The many different resources
available in the area and outside of the area will be made accessible to my supervisor and the
residents and their family members in a more convenient way. I will create a document that
will be alphabetically organized with all health care and adult protective services (APS). All
services will either be within the Pembroke area, and may expand out as far as Raleigh, to
better help those who are and those who are not from the Pembroke area. It will easily be
found on the computer, that way the social worker will be able to print out the information
for the residents and/or their family members with all of the resources contact information.
The social worker will be able to benefit from this organized solution by having an electronic
list of resources and she will be able to print out hard copies if need be.
