Per DA Administrative Order No. 23 series of 2012
What is ACCREDITATION?

- is certification of competence
In a specified subject or areas of
expertise, and of the integrity of
an agency, firm, group, or person,
awarded by a duly recognized and
respected accrediting organization.
Purpose of ACCREDITATION?
To met the organizational and program
service standards develop by impartial
consumers, stakeholder, municipal and
provincial organizations. It indicates that
the accredited organization has achieve
appropriate level of organizational
proficiency and that it has reliable
mechanisms in operation to continually
improve the quality of services it
delivers.
Objectives of DA RFO 12
ACCREDITATION;

( For NGO/ PO Accreditation)


1.To provide mechanism of certification
accreditation for NGOs/POs which met or
established minimum criteria for greater
transparency and accountability.

2. To encourage private sector participation in


social development through availment of
incentives under the comprehensive tax
reform program.
3.To simulate and integrate the efforts
of the non profit sector to elevate its
standards of service delivery.

4. To provide a system for greater GO-


NGO collaboration and
complementation.

I. Accomplished DA XII DA-APP FORM-A
(Application for Accreditation)
with letter address to RED thru Chairman
of RAEC/ DA-APP duly signed by the
authorized representative of NGOs/ POs
II. Accomplished DA XII DA-APP FORM-B
(Information Sheet)
- Profile/ background of the organization
- Economic Survey
a. Present Economic Activities
of the organization
b. Existing Activities
c. Proposed activities
- Organizations Capability
- Tract Record
- Proposed livelihood project
- Request/ intervention from DA
- Brief Description of Target Organization

14, 2012 and the DA Administrative
Order No. 23 series of 2012.

approved CBDP
(for LGU/ GO/ GA)
- Letter of Intent by LCE to avail
projects from DA
- Appropriate SP/SB resolution for
Fund Assistance and authorizing
LCE/ LGU to sign MOA as partner of
DA in project implementation.
- Certification from LCE herein of no unliquidated
funds downloaded from DA
- Project Proposal supported by approved LDP

ACCREDITATION COMMITTEE:

1. Receive, evaluate and process


application for NGO/PO accreditation
2. Issue Certificate of Accreditation to
qualified NGO/PO applicants
3. Mobilize, supervise, direct, control
accredited NGO/ PO.
4. Advice NGO/ PO under their super-
vision to apply for accreditation
REQUIRED PROCESS FOR
ACCREDITATION:
1. The NGO/ PO applicant must be endorsed
by the PAO/ CAO/ MAO thru proper channel
to the DA- 12 Regional Accreditation Committee
2. A Certificate of Accreditation shall be issued
to qualified NGO/ PO which shall be valid for a
period of two (2) yrs. unless sooner terminated/
cancelled for a cause.
3. Accredited NGOs/ POs shall be monitored
by the PAO/ CAO/ MAO/DA.
