the direct responsible provision of


medication related care for the purpose of
achieving definite outcomes that improve
a patients quality of life

a practice in which the practitioner takes


responsibility for a patients drug related
needs and is held accountable for this
commitment

At the heart it is about caring
Pharmaceutical care is :
a new professional practice
that has evolved from many years of research and practice
is not intended to replace the role of the physician or
any other practitioner but rather to meet a need in the health care
system

that has arisen because of :


multiple prescribers of medications for single patient,
the explosion of drug products and drug information
presently on the market,
the increased complexity of drug therapy,
the significant level of drug related morbidity and
mortality associated with drug use,
and the high human and financial cost of
drug misadventuring.
The principal elements of pharmaceutical care:

it is medication related,

it is care that is directly provided to the patient;

it is provided to produce definite outcomes;


(these outcomes are intended to improve the patients quality of
life )

and the provider accepts personal responsibility for


the outcomes
Medication Related
Pharmaceutical care involves not only
medication therapy but also decision about
medication use for individual patients.
As appropriate, this include decisions not to use
medication therapy as well as judgments about
medication selection, dosages,routes and
methods of administration, medication therapy
monitoring, and the provision of medication-
related information and counseling to individual
patients
Care
Central to the concept of care is caring, a
personal concern for the well-being of another
person
Overall patient care consists of integrated
domains of care including medical care, nursing
care, and pharmaceutical care
To pharmaceutical care, the pharmacist
contributes unique knowledge and skills to
ensure optimal outcomes from the use of
medications
Outcomes
It is the goal of pharmaceutical care to improve
an individual patients quality of life through
achievement of definite, medication- related
therapeutic outcomes
The outcomes sought are:
- Cure of patients disease
- Elimination or reduction of patients
symptomatology
- Arresting or slowing of disease process
- Prevention of disease or
symptomatology

Some tools exist now for assessing a
patients quality of life
A complete assessment of patients quality
of life should include both objective and
subjective
Patients should be involved, in an
informed way, in establishing quality of
life goals for their therapies
Responsibility
The fundamental relationship in any type
of patient care is a mutually beneficial
exchange in which the patient grants
authority to the provider and the provider
gives competence and commitment to the
patient
Responsibility involves both moral
trustworthiness and accountability

The Therapeutic Relationship
An integral component of pharmaceutical
care is the formation of the therapeutic
relationship between the pharmacist and
patient.
Since the patients need to be actively
involved in their own health care, it is
essential that they develop a trusting and
collaborative relationship with health care
providers.
In the therapeutic relationship, the pharmacist
forms a covenant with the patient: a promise
to do whatever possible to make sure the
patient achieves positive outcomes from
drug therapy.
Pharmacists contributions to this professional
relationship include:
1. Holding the patients welfare paramount
2. Maintaining an appropriate attitude of caring
for the patients welfare
3. Using their professional knowledge and skills
on the patients behalf
In this cooperative relationship, the
patients responsibilities include supplying
personal information, expressing
preferences, and participating in the
development of the care plan.
The relationship is facilitated by effective
communication, comprehensive data
collection, and emphasis on the patients
current and future well-being.
Easier Said Than Done
What does pharmaceutical care look like
when it is implemented in practice ???
It doesnt look like consultation
booths, pharmacists offices,
technicians, new computer systems,
or detailed patient charts, although
these are tools to facilitate
pharmaceutical care.
It is about truly having concern for patients and
spending the time and effort needed to help
another human being.
Actions the pharmacist takes might include
spending time with a patient to make sure
he really understands how to use his
dosage form, and could be placing a call to
the physician to discuss the
appropriateness of drug or dosage. Or it
could mean working with the patients care
providers to develop a system to assure that she
actually receives the agreed-upon medication
regimen.
The purpose of Profession
in Society
Profession are found wherever humans
live together in socialized groups.
Profession emerged in the Middle Ages
when specialized practitioners began to
develop and provide an array of
significant, unstandardized personal
services that were central to human
values.
Pharmacy is among the oldest of the healing
professions and its practitioners provide unique,
personalized services that meet fundamental
needs of individuals, communities, and society.
Profession means to testify on behalf of or to
stand for something.
People who are professionals stand for
something and avow not only to provide their
clients with knowledge but also to use a
particular body learning to solve a specific range
of human problems. In this context,
pharmacists not only profess to be experts
on drug therapy but also avow to help
people make the best possible use of
drugs.
