Definition
Hazard -> A Hazard is a potential source of harm or adverse
health effect on a person or persons
Risk -> risk is the likelihood that a person may be harmed or
suffers adverse health effects if exposed to a hazard
Injury -> physical harm or damage to someone's body caused
by an accident or an attack
Safety -> The condition of being protected from unlikely to
cause danger, risk, or injury

Accident Clasification
Near Miss is an undesirable event, it is hoped that if the circumstances are slightly
Near miss different it may result in injury to Humans, property damage or Process losses.

First Aid Case is a case of work accident that in the treatment of the wound does not
First Aid require the handling of professional medical personnel (nurse / doctor), just first aider
(P3K Officers) who have been given training.

Medical Treatment Case (MTC) is a case of work accident that requires wound care from
MTC professional medical personnel (nurse / doctor).

Restricted Work Case (RWC) is a work accident case in which the victim can not work
RWC normally on his part or is assigned to work in other types of work on the next shift after the
accident.
Permanent Partial Dissability (PPD) is a work accident case that causes the worker to be
PPD physically disabled / partial and permanent function such as loss of one eye / decrease of
vision function, one hand loss and the other.
Permanent Total Dissability (PTD) is a case of work accident which causes the worker to
PTD experience total physical disability and is permanent. Such as loss of vision, memory loss
and others.

Fatality Fatality is a case of work accident that causes death on the worker.

Risk assessment is the process of identifying and evaluating a hazard to determine the level of
action required to reduce a risk to an acceptable level.

Isolation Lock System (ILS)
ILS or Lockout-Tagout refers to the safety procedure used in industry and
research settings to insure that dangerous machines have been properly shut-
down and are incapable of being started up again prior to the completion of
maintenance or servicing work.
It requires that all hazardous energy sources have been
(1)identified
(2) isolated and
(3) rendered inoperative to prevent the release of potentially hazardous
energy prior to the start of any repair or maintenance procedure.
This is accomplished through the locking and tagging of all energy sources.
Some common forms of energy isolation include electrical circuit breakers,
disconnect switches, ball or gate valves, blind flanges, and blocks. Push
buttons, e-stops, selector switches and control panels are not considered
proper points for energy isolation.

Personal protective equipment, commonly referred to as "PPE", is equipment
worn to minimize exposure to hazards that cause serious workplace injuries
and illnesses. These injuries and illnesses may result from contact with
chemical, radiological, physical, electrical, mechanical, or other workplace
hazards. Personal protective equipment may include items such as gloves,
safety glasses and shoes, earplugs or muffs, hard hats, respirators, or
coveralls, vests and full body suits.
