DECs F0RM 1

( School register)
 This is a booklet form which contains different
information like the name of the school, district and
division where the school is located.
 It includes also the class, the grade level section and
the name of the teacher.
 Inside can be found the names of the pupils their
address, attendance, absences, or if they dropped from
classes.
DECs F0RM 2
 This is to be prepared certified true and correct by
the teacher.
 This contains teacher’s monthly reports of
enrolment and attendance.
 We call this ‘Daily attendance Report’.
DECs F0RM 3
 This is the Principal’s Report of Enrolment and Attendance.
 This report is prepared in three copies, one copy for the
School Principal, one copy for the District Supervisor, and
one copy for the Division Superintendent of Schools.
 This is a summary report of enrolment and attendance in
one school year.

 This is a summarized Report on Promotions
required by the Bureau of Basic Education.
 This is to be accomplished by the School Principal
or the Division Superintendent of Schools.
 It covers reports on the elementary and secondary
school levels including technical schools.
DECs F0RM18-a
 This is a Report on Secondary Promotion equivalent to DECS FORM
18-E-1 and 18-E-2 ( for the Elementary).
 This includes curriculum year and section names of students (surname
first and they are arranged alphabetically.) Home Address, Years in
School, Total number of Days of Attendance in the curriculum year,
Final Rating, Action Taken( Retained or Promoted) Total Age of
Students and initials of teachers to be approved by the Superintendent
and Principal.
 There is a due date of the submission to the Division Office indicated
in this form.
DECs F0RM18-e-1
(GRADES I-III)
 It shows the total age of pupils, grade average and
summary of failures and passers in the grade handled.
 This form is prepared by the teacher in four copies one
copy for the teacher, one copy for the school principal,
one copy for the district supervisor and one copy for the
division Superintendent.

 This is similar to DECS FORM 18-E-

 Except
that it pertains to Grade IV to
Grade VI pupils.
DECs F0RM 137-e
 This is the Elementary School Permanent Record a
confidential record in terms of specific data regarding the
individual pupil.
 This contains data on academic like character building
progress of the pupil from Grade I to Grade VI.
 A certificate of transfer is reflected at the last part of the
form.
DECs F0RM 138-e
 This is the academic achievement of the students per
grading period.
 A certificate of transfer is found on the front page.
 The teacher signed on this form, the Principal of the school
and signed by the parents for their comments at the end of
every grading period.
 It is returned for the respective teachers until the reports for
the whole year are accomplished.
DECs F0RM 178
 This is observation form report after the demonstration of a
particular teacher.
 His report is being done after the announced and
unannounced class observation.
 A copy of this form is given to the teacher concerned for
improvement of teaching if there is anything to be improved
in his teaching.
Cs F0RM no. 48
 This is a Daily Time Record. It is not DECS term but it is required in all
public schools.
 It shows the time that the teacher arrives and departs from school in the
morning and afternoon session.
 This is to be accomplished everyday.
 This form is handwritten.
 No other person is allowed to signed on this form except the one whose name
appears on the form is allowed to accomplish.
 This form is used in high school and elementary teachers in both public and
private schools.
