A questionnaire is a research instrument consisting of a

series of questions for the purpose of gathering information
from respondents.
Questionnaires provide a relatively cheap, quick and
efficient way of obtaining large amounts of information from
a large sample of people. Data can be collected relatively
quickly because the researcher would not need to be present
when the questionnaires were completed.
Questionnaires can be an effective means of measuring
the behavior, attitudes, preferences, opinions and,
intentions of relatively large numbers of subjects more
cheaply and quickly than other methods.

Self-administered Questionnaire
A self-administered questionnaire (SAQ) refers to a questionnaire that has
been designed specifically to be completed by a respondent without
intervention of the researchers collecting the data. An SAQ is usually a
stand-alone questionnaire though it can also be used in conjunction with other
data collection modalities directed by a trained interviewer. Traditionally
the SAQ has been distributed by mail or in person to large groups, but now
SAQs are being used extensively for Web surveys. Because the SAQ is completed
without ongoing feedback from a trained interviewer, special care must be
taken in how the questions are worded as well as how the questionnaire is
formatted in order to avoid measurement error.

Rating Scale
Ratings are subjective assessments that use a personal set of standards
to evaluate your product or service. Rating scale questions are used in
Satisfaction Surveys of all types such as: Customer Satisfaction Surveys,
Employee Satisfaction Surveys, Healthcare or Patient
Satisfaction Surveys, and Educational Surveys such as Student Evaluations
and Course Evaluations.
•They are subjective assessments that are not directly verifiable through
external observation.
•The response is based on personal standards and criteria rather than an
objective scale based on observable behaviors.
•They may vary from day to day and from situation to situation.
•They may be about issues the respondent has thought about and evaluated
carefully or about issues the respondent has not thought about or
evaluated at all.

Likert Scale
The Likert Scale is best used to measure and evaluate customer sentiment
on a specific product, service or experience.

should be consistent on each item; this prevents confusion for your
customers and simplifies the analysis of their answers for you.

The purpose of this question is to know the main cause of stress
among students of Accountancy, Business, and Management at Guisad Valley
National High School. That they can’t manage sometimes and they encounter
everyday that can cause stress to them.

These are different activities that can cause stress to ABM students:
• Having a lot of requirements / home works at the same time with a small
amount of time.
• Having conflicts with parents, friends, and significant other.
• Bullying
• Failing Grade
• Pressure of combining paid work and study.
• Difficulty in organizing work.
• Expectations (parents, teachers and society)
• Financial Problem

management strategies for the students and to know what appropriate
action they do to lessen their stress or manage their stress in a good
way.

• Eating Food or Binge eating
• Getting Enough Sleep
• Going outside/ Walking
• Watching TV/ K-drama / Movies
• Playing Instrument / listening to music
• Talking to someone about your problem
• Going to Church
• Self-meditation
• Playing Games (computer games, sports, and board games)
• Reading (Books, Newspaper, online books and magazines)

The purpose of this question is for the students if they are aware

life while studying
