chairman or president ( referred to as the Recipient).

 ARTICLE I: COVERAGE

ARTICLE 2. DUTIES AND RESPONSIBILITIES OF DA-RFO
12:
 Verify the legitimacy of the documents submitted by the recipient;
 Procure the machineries;
 Pay in full the cost of mechanization facility;
 Monitor/Inspect regularly the implementation of the project by the
recipient, including but not limited to the operation and maintenance of
the machinery;
 Provide extension services to the farmers that is dissemination of
knowledge, skills and technology (better farm practices );
 Turn over to the recipient the machinery through a certificate of turnover.
RIGHTS OF THE DEPARTMENT

Institute corrective measures against persons

responsible for the non-compliance of all the
mandatory terms and conditions of the MOA
and other laws applicable for the Rice Program

 Repossess the machinery

 DUTIES AND RESPONSIBILITIES OF THE
RECIPIENT
 SUBMIT ALL THE DOCUMENTS REQUIRED BY THE IMPLEMENTING GUIDELINES OF
THE PROGRAM
MINIMUM REQUIREMENTS:
1. Letter of intent and board resolution stating its need on the
technologies and its capacity to manage the project;
2. Organizations Certificate of Registration;

CDA CERTIFICATE OF REGISTRATION – COOPERATIVES
DOLE CERTIFICATE OF REGISTRATION – IA’S/FA’S
3. MAO/CAO/PAO Endorsement;
4. project proposal.

 Follow the operating scheme set by DA-RFO XII,

PHILMECH and LGU;

 Use the mechanization according to its intended

purpose
Make it easily available to its constituent rice farmer
Maintain a logbook reflecting the machinery’s
day to day operation;
Make it always available for DA-RFO XII
and other coordinating agencies
monitoring team, for inspection
Submit at the end of every harvest season a
facility operation report;
Be responsible for the safety and maintenance
od the mechanization facility;
 Be bound to surrender the mechanization facility under
the following conditions:

 Improper maintenance and utilization of the mechanization facility;

 Non-settlement of any conflicts arising from the use of the
mechanization facility;
 Written complaint duly validated BY DA-RFO XII arising from the use
of the mechanization facility;
 Non-compliance with the mandatory terms and conditions of the
MOA.
 AMENDMENTS
