For this phase, nurses generally refer to the evidence-based Nursing
Outcome Classification (NOC), which is a set of standardized terms and
measurements for tracking patient wellness.

S – Specific Type of outcome :


M – Measurable • Short-term goal – can be met
A – Attainable in a short period
R – Realistic • Long term goal – requires
T – Time frame more time
